The Post Holder will be part of the multi-disciplinary psychology Led Healthcare In-Reach team to provide evidence based physical health assessment and treatment to young people aged between 12-17 in a secure welfare setting, as part of a seven day a week rota service.

Main duties of the job

The postholder will assertively engage with young people through direct work as well as with the young person's local and wider professional network to identify and manage physical health needs of the young person.

The postholder will act as the health professional responsible in ensuring good physical health for the young person and improving future long term health outcome.

Job description Job responsibilities

  • To provide physical health nursing assessment and treatment to young people within a psychology led in reach health team daily.
  • Complete standardised specialist health assessments for children in secure care including information gathering from health agencies who have previously worked with young person.
  • Work within own competencies to refer young people to appropriate specialist services, including universal public health services.
  • To provide practical and emotional support to encourage parents and residential staff to take a proactive approach to the physical health needs of the young person.
  • To provide practical and emotional support to encourage young people to take a proactive approach to their own physical health care.
  • To work in partnership with parents and residential staff within the individual psychological formulation for the young person
  • To lead on sharing holistic health needs to professional meetings centred around the young person
  • To lead on comprehensive health care plans developed in partnership with the young person which will include mental health, physical health and emotional wellbeing.
  • Maintain accurate and comprehensive clinical records and notes and complete reports to a high standard.
  • Direct work with the young person to promote independence, develop life skills, and support safe transition from Clare Lodge.
  • Identifying and referring to appropriate health services as part of the young persons transition from Clare Lodge into a community setting.


Person Specification Education / Qualifications Essential
  • Registered General Nurse/Children's Nurse
  • Advanced Clinical Assessment Skills/Minor Illness or Injury or interest to complete on employment.
  • Non-medical prescribing or interest to complete on employment.
Desirable
  • Specialist Community Public Health Nurse.
  • Knowledge of specific needs and risk for children in care and related conditions.
  • Provision of therapy intervention acquired through training and experience
  • To have completed appropriate Child and Adolescent mental health training or be committed to complete on employment.
  • Sexual Health and Contraceptive Training.
Experience Essential
  • Experience of working with vulnerable children and their families
  • Experience of working with young people with complex health needs.
  • Experience of physical health assessment and medical triage
  • Experience of safely managing minor illness and injury and development of treatment plan.
Desirable
  • Experience and underpinning knowledge of child and adolescent mental health and related difficulties including substance misuse and self-injurious behaviour.
  • Delivering group sessions
  • Experience delivering training.
  • Experience of complex carecoordination with multiagency partners.
Skills & Abilities Essential
  • Understanding of needs of children who present with high risk behaviour or interest to complete on employment
  • Venepuncture or commitment to complete on employment.
  • Able to demonstrate commitment to principles of high-quality care and service provision
  • Good level of IT skills
  • Standard keyboard skills and ability to communicate through IT using packages such as Word, Office
