We have a fantastic opportunity for an NMC registered nurse.

As a Children and Young People's Case Manager, the post holder will work with the ICB, system partners and with NHSE in delivering an effective service to ensure that safe and timely discharge plans are in place and that community packages of care and support are reviewed and evaluated in a timely manner.

To participate in Care, Education and Treatment Reviews, (C(E)TRs) as required to ensure timely robust discharges and to avoid progression of clinically inappropriate admissions.

To work with other Case Managers in the management of cases that would benefit from support in breaking down barriers to discharge, with emphasis on best possible outcomes for children and young people and their family.

To provide clinical support and guidance to commissioners in relation to the defined population, future service needs, identifying themes and trends.

The post holder may be required to attend Mental Health Tribunals, CPA meetings, acute care facilities and secure units.

Main duties of the job

Where an individual is in hospital/52-week residential placement the post holder will be required to regularly present comprehensive written reports providing assurance to the ICB and provider Trust that the agreed placement is delivering identified outcomes. Likewise, when an individual on the caseload is in a community placement the post holder will liaise with commissioners, CHC and local authority to ensure the care being delivered is of high quality, safe and is outcome-focussed.

• To commission and review safe and cost-effective individual packages of care for Children and Young People with mental health, learning disability, physical health and autism.
• To oversee the cases of Children and Young People in inpatient and/or community care with complex needs including mental health, learning disability, physical health and autism.
