The role of the Clinical Nurse Specialist (CNS) will ensure that the care delivered by the Complex Care Support Team (CCST) will be evidence based and provided in a compassionate and caring way.

The role will support the importance of Quality and Professional standards within the team and ensures that the delivery of high quality care is consistent with the CQC fundamental standards, Safeguarding principles, Trust values and the Trust and operational strategic objectives.

Main duties of the job

The post-holder will be responsible for ensuring all team members participate in audit, evaluation and research. To support the team in achieving positive clinical outcomes for patients and their families. To be the lead clinician in training, education and development.

To offer advice signposting for patients, in collaboration with other members of the MDT, so that patients that have mental health conditions can be treated in primary care.

About us

Croydon Outcome Based Commissioning Alliance has developed six Integrated Care Networks (ICNs) across the borough. A central feature of the ICNs is the advent of 'GP huddles' which ensure that patients regarded as medium to high-risk are identified quickly and proactively case managed as a result of this. The team is based at Heavers Resource Centre but will primarily work within ICNs.

Key Responsibilities:
  • To provide senior nurse leadership to team members and continued support to the team manager. To provide specialist Mental Health of Older Adults and Dementia evidence-based interventions to older adults with complex needs that meet the CMHT criteria.
  • To attend weekly GP huddles and offer comprehensive initial assessments and short term case management for patients in collaboration with other members of the MDT.
  • To work with the MDT to ensure that the care offered is evidence based.
  • To signpost patients under the age of 65 following assessment to relevant SLaM services.
  • To effectively assess and manage complex psychiatric and physical health risk as required. To maintain accurate clinical documentation and complete outcome measures as per Trust policy.
  • Ensure effective verbal and written communication with all staff, service users and carers in order to maximise user and carer involvement in the delivery of care and treatment.
  • Raising safeguarding alerts and working with the Directorate/Trust safeguarding leads.
  • To work with partner agencies and other organisations within the huddles that will encourage people to be supported at home. To work in a manner that supports effective partnership working with all services that will enhance service user and carer interventions.
  • Provide primary care team practical advice, including phone advice, on how they can support/manage patients with mild-moderate mental health problems that are impacting on their mental health.
  • To undertake training and development as required in line with the responsibilities of the post and the Trust policy.
  • To actively promote the workplace as a learning environment thus ensuring best practice.
  • Responsible for supporting the Community Matron and Heads of Nursing in assuring the quality of nursing practice in the complex care support team.
  • To provide regular supervision to junior members of staff and participate in the Trust appraisal system and PDP. To adhere to the relevant professional regulatory councils.
  • To adhere to health and safety policy and legislation. Adhere to the relevant Trusts policies in particular; Infection Control policy and health and safety policy and legislation.
  • To ensure the safety of self and others in reference to the lone working policy.
  • To provide clinical cover for team members in their absence. To undertake any other reasonable duties as requested by Line Manager/Service Management.


Person Specification Education/Qualification Essential
  • RMN Nursing qualification Degree/Diploma with current NMC registration
  • Training in clinical supervision such as ENB 998/mentorship or equivalent
  • Evidence of ongoing professional development
Desirable
  • MSc or working towards
Experience Essential
  • Relevant experience of community psychiatric team/home treatment team/liaison
  • Experience and ability to function as a specialist practitioner with advanced clinical expertise in working and caring adults with complex needs and their carers in a community setting
  • Evidence of good practice in holistic assessments
  • Evidence of receiving and providing clinical supervision
Desirable
  • To have experience working with older adults with dementia or with mental health problems
  • A good working knowledge of mental health disorders that affect older adults
Knowledge Essential
  • A good working knowledge of mental health disorders that affect adults
  • Good working knowledge and competency in physical health screening and monitoring
  • To have knowledge of information governance
  • A good working knowledge of relevant mental health registration
Desirable
  • Evidence of using safeguarding adults/children in practice effectively in older adults
Skills and abilities Essential
  • Assessment of risk and management of risk factors including positive risk taking
  • Knowledge and understanding of the recovery and support model
  • Evidence of competency in clinical judgement
  • Evidence of using safeguarding adults/children in practice effectively
  • Ability to teach and develop staff

  • Proven ability to work within a changing environment
