An exciting opportunity has arisen for Band 7 Clinical Lead in the Solihull Primary Care Networks.
Within BSMHFT our community mental health services are well-established and as partof the Birmingham and Solihull community transformation programme, we are looking to expand our primary care teams and primary care practitioner roles. This is an exciting opportunity to be at the heart of national change and development in the delivery of community mental health care, in line with the NHS Long Term Plan.
Main duties of the job
The Clinical Lead will work across the Community Primary Care Mental Health Hub () providing, clinical leadership and clinical expertise. To be responsible for the development and maintenance of effective clinical pathways between Primary care and other services. To provide additional leadership and guidance to other team members in risk assessment, risk management and positive problem solving and lead on debriefs following incidents. Develop confidence and competence within the team for managing complex clinical cases and issues. To take a lead role in the provision of clinical leadership and clinical supervision to the team.. To lead on the management of referrals to the team and ensure that the team undertakes assessments and on-going intervention, and undertake the duties and responsibilities of a lead worker and maintain an active lead worker caseload . To work collaboratively with multi agency partners to produce clinical management plans.. To ensure the provision of the highest possible standard of care within the PCL team and ensure the team follow these principles Ensure patients within the team have an up to date integrated care record, reflective of their risk and care needs.. To carry a case load determined by Hub Manager
