Liverpool Core 24 forms part of the 24/7 Urgent Care Model and urgent care pathway within Mersey Care NHS Foundation Trust. The 24/7 Urgent Care model provides mental health assessment, intensive community interventions, advice, support and guidance twenty four hours, seven days a week across several access points for individuals presenting in crisis. Mersey Care NHS Foundation Trust has brought all urgent care teams together to create one central hub and thus diverts service users away from blue light services and affords them the right access to the right care at the right time in the most appropriate environment. The components which are part of the 24/7 Urgent Care Model include Mental Health Hospital Liaison, Criminal Justice Liaison Teams, Street Triage Cars, 24/7 Urgent Care Hub, Clinical Decision Units and Crisis Resolution Home Treatment Teams.

Mental Health Liaison is an integral part pf the 24/7 Urgent Care Model and this is a great opportunity for a motivated individual with an interest working in an acute hospital environment to expand their knowledge in both crisis intervention and physical health care knowledge working in the urgent care model of diversion of service users away from AED to the Clinical Decision Unit or, to undertake assessments of individuals who present to the AED or are referred from an acute hospital ward.

Main duties of the job

1. To coordinate the care of service users carrying out assessment planning,implementation and evaluation of their care under the guidelines of the care programme approach and under the supervision of the line manager.2. To ensure that the service users care plan is developed to meet the assessed need of the individual and that unqualified staff are delivering care as identified in the care plan.3. To liaise with family members, carers and significant others when completing the assessment and developing a care plan.4. To undertake and supervise risk assessments and formulation of risk management plan for service users under your care, taking note of the contributions from other members of the team, family members carers and significant others.5. To ensure Safeguarding Children and Adults requirements are followed and participate in meetings as appropriate.6. To direct unqualified staff in order to ensure delivery of a high standard of care to service users.7. To participate in setting standards for care based on sound evidence based and research findings.8. To participate in the systematic monitoring and evaluation of service users.9. To communicate to other members of the multi-disciplinary team information which has an impact on the service user's care.

The Hospital Mental Health Liaison Team practitioners work alongside the multidisciplinary team within the acute hospital setting including medical and nursing teams, Community Mental Health Teams, Local Authority community services and Voluntary sector.

The role will serve to identify and support patients who present within the acute hospital with a variety of mental health conditions alongside the potential of complex physical comorbidities, individuals can present in crisis and display challenging or vulnerable behaviours who require a comprehensive mental health assessment to support and manage mental health problems and to contribute to the discharge planning process.

Practitioners will have the opportunity to work across all ages from 16+ to older age and across both A+E and ward environments.

We are seeking adaptable, flexible, innovative and competent mental health practitioners who have comprehensive experience of working in mental health services and wish to develop their professional skills within the hospital setting. This will involve completing mental health assessments and the ability to effectively signpost while forging links with community organisations across health and social care. You will build and maintain effective relationships with health professionals in the general hospital settings to raise awareness with regard to individuals with mental health issues. You must be able to work independently with confidence in order to make informed clinical decisions regarding appropriate signposting/discharge process.

You should have a Registered Mental Health Professional qualification, with significant experience of working within the field of Mental Health.

Person Specification QUALIFICATIONS: Essential
  • Registered Mental Health Nurse or Dip.SW/CQSW, Dip.COT/SROT
  • Evidence of post registration study / continuing professional development e.g. ENB 998 / Mentorship in Practice.
EXPERIENCE Essential
  • Significant demonstrable post registration experience. ASW/ Mental Health Practitioner
Desirable
  • Evidence of experience of working a range of people with mental health problems in a variety of health care settings
  • Experience of Crisis Intervention
  • Experience of leadership and management.
KNOWLEDGE & SKILLS Essential
  • Able to communicate effectively both verbally and nonverbally.
  • Possess excellent listening and negotiation skills
  • Leadership skills
  • Ability to work as a member of a multidisciplinary team.
  • Experience of Mental Health assessments
  • Demonstrate a good knowledge of Code of Conduct and the nurse's responsibility to identify gaps in their knowledge base
  • Demonstrate knowledge and experience in the concepts of mentorship, preceptorship and supervision and ability to provide education and supervision to qualified and unqualified staff.
  • Knowledge of developments in community care and of mental health legislation.
  • Knowledge of CPA guidelines.
