An exciting opportunity has arisen within our Memory Assessment Services in Rushden and Daventry.
The successful applicant would be working within an enthusiastic and dedicated team providing comprehensive memory services to older adults over 65 years of age. In conjunction with a well established multi disciplinary team comprising of Mental health nurses, support workers, Clinical psychologist and Consultant, the successful candidate will be responsible for managing their own caseload and coordination for individuals who are experiencing memory difficulties. This will involve working very closely with other key stakeholders and families.
The successful candidate will work as a member of the specialist Community Mental Health Team, ensuring the highest standards of care, by maintaining the quality of own work by developing skills and knowledge, making changes to practice and contributing to the development of others within the team.
As a member of the staff you will be expected to supervise and support junior practitioners/students within a designated locality.
Contribution to service improvement is a necessity of the role, through participation in implementation of improvements and changes to practice.Within the workplace the individual must monitor and maintain the health and safety of both self and other and also support equality and diversity.
Main duties of the job
Provide full and detailed assessments for those experiencing memory problems with full involvement of family members.
The role involves managing a caseload with the need for prioritisation and organisation skills. Keeping up to date with KPIs.
Contribute to the Multi disciplinary team with discussions to support management of individuals.
