We're looking for a Care-Coordinator to work in the community as a part of the Multi Disciplinary Adult Mental Health Team in Wallingford/Henley-on-Thames and Thame.

We are welcoming applications from both experienced Nurses/OTs and newly qualified Nurses/OTS seeking developmental opportunities, if you were coming into a developmental role, you will be provided with increased supervision and training. The expectations around complexity and autonomy will be less (please state this within your application if you are interested).

Main duties of the job

This vacancy is for our Treatment Team, which means you will predominantly be working with people as their allocated Care Coordinator under the Care Programme Approach. In this role you will be central to building relationships with the people on your caseload and working alongside them towards planning and implementing their recovery goals.

You will need to be able to work autonomously. You will have experience with the CPA process and able to assess and plan treatment with service users. You will also be able to critically assess and manage risk, including the use of Safeguarding processes.

There are opportunities for team working and facilitating groups such as MBT/CBT based interventions. The MDT consists of medics, psychological therapists, social workers, OTs, RMNs and support workers, as well as embedded workers from the third sector to support with employment, housing and social needs.

In addition, we are currently embedding Peer Support Workers within the teams to promote the power of lived experience and ensure that the gap between patient and clinician is bridged.

We will need you to work preferably 30 to 37.5 hours per week with some weekend working - we welcome flexible working so please call us to discuss how we may be able to accommodate other working patterns. You will not be required to work night shifts.

  • New starters have a 6 month probationary period. This, together with the induction process aims to create a positive supportive working environment allowing new employees to learn key elements of their role over a reasonable timescale.
  • Candidates not currently employed by the Trust who attend an interview for non-qualified Band 1-5 posts are required to undertake numeracy and literacy assessments.

Person Specification Education and Training Essential
  • Registered Professional Qualification (Mental Health Nurse , Occupational Therapy)
  • Evidence of Continuing Professional Development (CPD) portfolio
Desirable
  • Post graduate education specifically relating to client group (Post basic qualification in community care/ research awareness/ management)
Occupational Experience Essential
  • To have experience of working within a multidisciplinary team
  • To have relevant experience in Adult Mental Health care
Skills required - IT / Admin Essential
  • To be IT literate
  • To have excellent English written communication skills
