We have a brand-new opportunity for a Mental Health Nurse to join the Crisis Resolution & Home Treatment Team (CRHTT) at Eaglestone Health Centre, Milton Keynes.

For most people with mental health needs, the diagnosed problem isn't all they need or want help with. People have said they need mental health services that go beyond symptoms and illnesses; joined-up services that see a person rather than a diagnosis and that work together to promote their all-round well-being.



The CRHTT team is a part of acute care and works closely with the acute hospital and community teams to support service users in a mental health crisis. It offers service users care and support in their own homes as an alternative to hospital admission. CRHTT are able to offer all the benefits of a hospital admission while remaining at home including home visits from our experienced team, with support from doctors and psychology. We also facilitate supportive discharge from hospital, managing the transition back to community-based service.

Main duties of the job

The post holder will provide direct care, support and treatment to acutely mentally ill service users in community settings seven days a week. The role involves rapid response, face-to-face psychosocial assessments, support and identification of appropriate care pathways of those with various mental health problems and in a mental health crisis. The role holder will participate in gatekeeping of admissions to hospital and facilitate discharge from inpatient settings once safe and appropriate. The post holder will refer and signpost service users to appropriate agencies identified according to service user needs and care pathway. Referrals may be from GPs, first response service, inpatient services, psychiatry hospital liaison team, AMHPs and other adult mental health services.

Person Specification Qualifications Essential
  • Professional Qualification at Diploma or Degree level in Mental Health nursing or equivalent Occupational Therapist qualification
  • Valid registration with governing body is essential.
Experience Essential
  • Post basic experience in Mental Health and evidence of continuing professional development.
  • Working with users who have severe and enduring mental health problems in ether an inpatient or community setting
Desirable
  • Experience of undertaking complex health and social care assessments
Knowledge Essential
  • Excellent knowledge of MHA 1983 as amended by MHA 2007, the MCA 2005 and the Care Act
  • Knowledge of assessment and risk assessment tools.
Desirable
  • Active in MDT approach.
Other Essential
  • Car owner with a valid driving license and a willingness to travel to different locations to undertake role effectively
