The permanent posts are eligible for a £1500 New Hire Bonus. An initial payment of £500 will be paid in the first 1 month and a final payment of £1000 will be paid 1 year after your start date. This will be pro-rata for part-time posts. Internal applicants are not eligible.

We are seeking an enthusiastic band 5 Occupational Therapist to join the Integrated Stroke Early Supported Discharge and Community Neuro Rehabilitation Service. The Integrated service provided both stroke rehabilitation and community neuro rehabilitation supporting service users with a variety of complex needs. The Early Supported Discharge Service (ESD) is a key pathway for stroke survivors allowing them to leave hospital earlier than they may have otherwise; they receive up to 6 weeks of intensive rehab in their home / community or in one of our inpatient Community wards in Cambridge or Peterborough.

This is an exciting opportunity to join an established multi-disciplinary team comprising of Occupational Therapists, Physiotherapists, Speech and Language Therapists, Psychologists, Specialist Nurses, and Neuro Rehab assistants. This role requires the successful candidate to manage a clinical caseload across the South of Cambridgeshire. Their base will be either Huntingdon or Cambridge dependent on location nearest to home.

Main duties of the job

You will provide comprehensive assessments and deliver evidence-based specialist neuro occupational therapy programmes. You will work as part of a multidisciplinary team in supporting patients with physical, cognitive, and psychosocial needs to achieve their potential.

The successful post holder will receive and provide clinical supervision and an annual appraisal.The ideal candidate should be flexible and adaptable to support service delivery.

The role involves visiting patients in their homes, wards, or community gyms. There may be times when there is a requirement to provide clinical cover elsewhere within Cambridgeshire. The role requires agile working often using virtual consultation to manage a caseload. We will provide a smart phone and laptop to support this role.

The successful post holder must be able to travel independently across their work locality, have a valid driving licence and access to a car for work purposes.

  • To work within the Trust, RCOT and HCPC guidelines, demonstrate a good working knowledge of nation and local standards of practice in line with legal and operational requirements and use own professional judgement to vary form established practices where required.
  • To promote independence and wherever possible to avoid hospital admission and reduce inpatient length of stay to ensure those patients receive the appropriate care in the most appropriate setting.
  • Organise and manage own time, delegating work appropriately.
  • To conduct and contribute to risk assessments and health and safety assessments and including immediately reporting any changes/newly identified risks.
  • To be responsible for the implementation of resulting recommendations from risk and health and safety assessments.
  • To demonstrate the importance of gaining patient and carer consent in all interventions.
  • To effectively communicate verbally with patients, carers and colleagues using tact and persuasive skills. This may involve using skills where patients have difficulties in communication, e.g. hearing loss, diminished sight, depression, speech problems, cognitive impairment, behavioural problems and pain.
  • To contribute to and work towards the service/organisational aims and objectives of the Integrated ESD and CNRS.
  • To work effectively within the Integrated ESD and CNRS structure and liaise appropriately with members of the MDT and other agencies by attending MDT meetings, case reviews.
  • To ensure that accurate and timely written records are kept which comply with the Trust policy and HCPC guidance, reporting on any issues as appropriate.
  • To inform/update to all members of the MDT, service users and appropriate others of changes involving current care plans, patients progress and other relevant matters that pertain to the care of the patient.
  • To support the Specialist Practitioner with day to day planning and delivery of a high quality service.

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence* and have use of a vehicle. You cannot usepublic transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Person Specification Education / Qualifications Essential
  • Degree or equivalent, e.g. BSc, or Diploma entry level qualification into the Occupational Therapy profession.
  • Current HCPC registration.
  • Documented evidence of own continuing professional development.
Desirable
  • Member of Royal College of Occupational Therapy
  • Member of Royal College of Occupational Therapy Specialist Section Neurological Practice.
Experience Essential
  • A broad range of undergraduate placements completed including stroke and/or neurology.
Desirable
  • Clinical experience including neurology and/or stroke.
