The community is the place to be! Rehabilitation in the community is at the forefront of the implementation of Discharge To Assess. You may have gained experience in the acute hospitals and now your career in the community beckons. This role is based in the therapy led Intermediate Care Service where you will work with the wider multidisciplinary team to rehabilitate patients, discharged from hospital and to prevent admission, in their own homes and in care homes. You will be based in one of our locality teams but work closely with your colleagues in other localities and across the system that provide Intermediate Care for the population of Cambridgeshire and Peterborough. We offer excellent professional support, and you will have the opportunity to rotate to other specialist teams within CPFT such as in-patient rehabilitation, neuro and neighborhood team community therapy.

As part of the role, you will:
  • provide assessment and physiotherapy interventions to patients in their own homes and care homes.
  • implement therapy plans as well as delegating to therapy assistants and integrated care workers.
  • develop your skills in sub-acute rehabilitation and discharge planning in the community working withcolleagues in health, social care and the voluntary sector.
  • work autonomously in the community setting travelling across the community seeing people in their own homes

  • To work towards the acceptance of clinical responsibility and professional autonomy for a designated caseload of patients, from referral to discharge.
  • To be professionally and legally responsible and accountable for all aspects of your own work, including the management of patients in your care.
  • To accept responsibility for ensuring the effective selection, use and fitting of equipment.
  • To independently plan and carry out assessments in the patients own
  • To take responsibility for appropriate delegation of tasks to therapy.

  • You will need to hold a full UK driving licence* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Please confirm in your application that you meet the specified criteria.

    Person Specification Education and Qualification Essential
    • Degree or equivalent, e.g. BSc, or Diploma entry level qualification into the Phsyiotherapy profession.
    • Current HCPC registration
    • Documented evidence of own continuing professional development
    Desirable
    • Member of CSP
    Experience Essential
    • Experience of working in the NHS
    • A broad range of undergraduate placements completed in a variety of settings.
    Desirable
    • Clinical experience working with older people and long term conditions.
    • Experience of working in the community.
