As part of our commitment to improving patient flow and length of stay We are seeking a full-time band 6 working in the patient flow team as a Clinical Inpatient Admission to Discharge facilitator. The role has been developed to support inpatient services and external stakeholders in assuring patients are discharged in a timely manner.

The team base will be at the Caludon Centre. There is an expectation to be able to travel across all mental health inpatient sites.

The ideal candidate will have a strong team work ethic, sense of accountability, accompanied by a 'can-do' attitude towards their work and a strong values base that drives practice. Confident in their clinical knowledge. The successful candidate will be flexible and have the ability to travel between sites. This post is open to candidates with the right attributes to enhance our team.

The role will require the post holder to ensure that the wards are reporting patient flow and detailed discharge plans through the mental health inpatient bed management and delayed transfer of care governance reporting system and those patients have a discharge date at the point of admission.

To ensure that all patients deemed fit for discharge are reported accurately, and managed and escalated within the agreed identified processes and procedures.

This post will cover Monday to Friday between 9am to 5pm.

This is a permanent full time post.

Main duties of the job

The post holder will take delegated responsibility for the escalation of specific bed management issues to the relevant clinical unit or department. Working alongside the multidisciplinary teams for each area the postholder will work innovatively and creatively to find solutions to individual issues, including identifying complex discharge issues that need to be escalated to the respective clinical or management teams.

The role will require the post holder to ensure that the wards are reporting patient flow and detailed discharge plans through the mental health inpatient bed management and delayed transfer of care governance reporting system and that patients have a discharge date at the point of admission.

To ensure that all patients deemed fit for discharge are reported accurately, and managed and escalated within the agreed identified processes and procedures. This will include assuring reports are undertaken in a timely manner and provided to the identified panel, professional body, and scrutiny panels.

The post holder will also be required to support clinical staff in presenting reports at funding panels.

The post holder will be required to oversee acute out of area admissions, attending MDT's, ward rounds, and supporting the discharge pathway for patients placed in private hospitals, ensuring barriers to discharge are raised in a timely manner and length of stay is not prolonged unnecessarily.

Job description Job responsibilities

The post holder will be responsible for the provision of reliable, consistent, and timely information on the mental health bed state, identifying capacity and demand situations and working to support the senior members of mental health services to evaluate and manage clinical risk related to length of stay, delayed transfers of care within the inpatient settings and acute out of area admissions. To support the resolution of these issues and assist to manage the patient flow and acute out of area admissions.
