The Primary Liaison Nurse role in the Acute and Primary Learning Disability Liaison Service is an opportunity for someone with ambition and drive to extend the scope of their practice. The admin base is negotiable, either within West or East Surrey. The successful candidate will be expected to cover both areas, alongside our other PCLNs. Your main area to cover will be Surrey Downs.

We are looking to recruit a Band 6 Nurse for (0.6 WTE) a week to join the service. We welcome applicants who are RNLD, RMN, or RGN qualified with Learning Disability experience.

Travel is required to other parts of the County on SABP business. You will be required to work autonomously, as part of the wider team and with a variety of agencies outside of the Trust.

Main duties of the job

The successful candidate will be enthusiastic about promoting Annual Health Checks. The team is community based, working closely with GPs to identify individuals with a learning disability providing additional support, around improving access to health care by promoting reasonable adjustments.

The successful candidate will need to be passionate about reducing health inequalities and improving the health needs of adults with Learning Disabilities. They also work closely with people with learning disabilities and their families and carers.

An essential part of the role is to create and deliver training to Primary Care and the wider networks

The team are working with a range of partners on initiatives to increase uptake of Cancer and health screening for people with Learning Disabilities. This is an exciting area for development that the successful candidate would be expected to be a part of. You will need to be highly motivated, imaginative and committed to health promotion.

There will be an expectation for the successful candidate to work alongside the Trusts social care homes and with other social care providers within a designated geographical area, to ensure equal access and positive health outcomes and to increase engagement between the Trusts social care homes and GP surgeries. The successful candidates will be supported to retain and manage their own case load and project work.

The post is with the acute and primary care team, which is part of the specialist learning disability health care services. The post holder will be based within one of the community Learning disability teams but will be required to consider a Surrey wide approach and to work flexibly.

Person Specification Experience Essential
  • 2 years of relevant post qualification clinical experience/ management of a case load.
  • Experience of participation in clinical practice/ service development / project work.
  • Experience of supervisory skills.
  • Evidence of developing experience in a specialist interest area.
  • The candidate must have a full and valid driving licence due to the community element of the role and the large geographical area that the liaison service cover.
Qualifications Essential
  • Nursing degree.
  • Additional academic qualification relevant to area of work.
  • Nursing mentorship or Teaching and Assessing in Clinical Practice qualification.
  • Leadership and or Management qualification / evidence of study.
