Community Psychiatric Nurse- Assessment Services coordinator

An exciting opportunity for a CPN to work across Newport East and Newport West CMHT, focusing on assessment parts of the service, taking the lead within routine and urgent assessment clinics, to coordinate and take part in the assessment process. For the individual to work closely with the team leads to take forward an ongoing assessment pilot.

We are looking for an individual who is interested in strengthening existing links with CAMHS services to aid the transition for service users from CAMHS to adult services.

The successful applicant would also be expected to represent the CMHT at multiple agency meetings.

Main duties of the job

This is an excellent development opportunity for a Band 6 Nurse with post registration experience to be supported in developing community nursing and assessment skills in an established and evolving integrated community mental health team. You will have knowledge and experience in the care of people with complex, severe and enduring mental health problems in a community setting. You will have the opportunity to link with the wider service which includes the CMHT clinics, CRHTT, AOT and the inpatient ward.

You will be committed to ensuring the delivery of the highest quality user-centred care and contribute to the development of adult mental health services in Newport. You will be responsible for coordinating and taking the lead in urgent and routine assessments. In addition to supporting the transition of service users into adult secondary mental health care. The successful applicant will be part of a developing service providing a high standard and evidence based care to those in Newport.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
