Community Mental Health Nurse (Band 5 Nurse)

Are you ready to step into a pivotal clinical role in secondary mental health services?

The posts will be based either at Millwood Unit in Uckfield or Hill Rise in Newhaven. Both sites are set in the beautiful rural area of East Sussex in the South Downs National Park. The Assessment and Treatment Service offers a wide range of evidence-based treatments for people with severe and enduring mental health problems.

As a Community Mental Health Nurse in our service you will be focussed on improving outcomes for service users, working creatively with them and their carers to co-create care and risk plans that will help them achieve their goals and live as independently as possible. The key to success in this role is often about building effective partnerships with GPs, Crisis services, housing and other third sector providers, as well as making decisions to refer patients onto one of our more specialist services, such as our Early intervention in Psychosis or Eating disorders.

To protect the health and safety of our staff/workers, patients, and those attending our sites, we encourage our workforce to get fully vaccinated against COVID.

Main duties of the job

To be successful as a nurse at Hill Rise ATS you will be:
  • A Registered Nurse
  • Excellent at forming relationships with our partners
  • A service-user focussed practitioner

It would also be advantageous if you have:
  • Experience working with adults with severe mental illness
  • Experience working in a county-wide community team

Job description Job responsibilities

1. To be an active participant in the multi-disciplinary team, formulating care plans and utilising appropriate assessments.

2. To take part in all quality initiatives actively supporting and participating in both policies and service developments in the specific care group.

3. Carry a caseload, actively working with parents and carers.

4. To participate in regular clinical, professional and managerial supervision.

5. To maintain and contribute to standards of professional care, ethics, policies and procedures.

6. To contribute, to improve access for patients to local generic health care services, primary, secondary and specialist services.

Person Specification Qualifications Essential
  • Registered Nurse with current registration [mental health/LD/Adult], appropriate to the job role.
Knowledge Essential
  • Knowledge of MHA (1983/2007).
  • Knowledge of Mental Capacity Act (2005), to include practical knowledge of FACE documentation, DOLS and Best Interest decisions.
  • Child and Adult safe guarding procedures.
  • Change management at organising and prioritising their workload.
  • Demonstrate ability to enhance care through evidence based knowledge.
  • Demonstrate professional development relevant to the specific care group.
