Somerset NHS Foundation Trust have an exciting opportunity for a full time Band 5 Learning Disability Community Nurse to work within a multi- disciplinary team, working collaboratively with other agencies to support people with learning disabilities and autism with accessing services to meet their health care needs.
We are looking for a newly qualified or more experienced nurse who wants to further their experience, skills in nursing and support with service development. Support will be given through; regular nurse meetings, clinical and management supervisions as well as peer discussions. Opportunities will be given for professional development.
Main duties of the job
Newly qualified nurses will be given the opportunity to complete preceptorship, under direction of a nominated supervisor.
This post will hold various duties:
- Manage a caseload of patients under indirect supervision and be accountable for assess, planning, implementation and evaluate episode of care, within professional guidelines.
- MDT working to support safe, effective care.
- Working collaboratively within the Specialist Community Learning Disability Health Team, building relationships with other community, primary, secondary and tertiary services/agencies.
- Have an understanding of legislations, policy's and procedures.
- To other clinical/management supervison to non registered colleagues.
