Assertive Outreach West is seeking a registered Mental Health Nurse to join our well established team in meeting the needs of our service users. If you are an RMN with experience working with people suffering with mental ill health whether this be inpatient or community based we would love to hear from you.

Assertive Outreach team are able to offer exciting opportunities for either a Band 6 Community Mental Health Nurse or newly qualified staff at Band 5 with a view to gaining training, experience and support over a set period and with the opportunity to move into a band 6 post on successful completion.

You will work as part of an established integrated multi-disciplinary team made up of Psychiatrists, Psychologists, Mental Health Nurses, Support Time Recovery Workers, Occupational Therapists.

This post has become available at a very exciting time following the Rehab review. There has been significant investment available to review and expand the Assertive Outreach Team covering pan Dorset.

The Assertive outreach Weymouth team covers pan Dorset County, with the exception of Bournemouth, Poole and Christchurch.

You will need to have a valid driving licence and use of a car as you will be required to assess and monitor patients in their own homes

Main duties of the job

The primary role responsibility is being a dedicated Care Coordinator for a caseload of patients with whom you'll provide person centred targeted support to assist in aiding their holistic recovery. There are opportunities for you to advance you own clinical assessment skills whilst risk assessment will remain at the forefront of everything you do. Alongside this there will be duty working and assisting in the completion of regular clinics i.e. depot, Olanzapine and physical health. You should have recent experience of working in a mental health environment and will need to be able to apply the philosophies of engagement and recovery in your work with service users.

It goes without saying that you are reliable, committed and motivated with excellent communication, organisational skills with the ability to prioritise. In return we are ready to offer you comprehensive structured management supervision, management you can approach any time and peer support which is second to none. In addition to this we are also happy to support you in advancing your own skill set using additional learning opportunities such and NMP, ACP and psychological therapies.

The post is for 37.5hrs covering Monday to Friday, 09:00hrs to 17:00hrs.

  • Band 6 - Significant post registration demonstrable experience within mental health services, working with people with severe and enduring mental illness and complex needs . Band 5 and 6 Knowledge of the legislative framework Able to demonstrate the ability to clarify the therapist role in the team. . - A clear understanding of the function of a rehabilitation service and a demonstrable knowledge of therapeutic interventions used within the rehabilitation service . The ability to provide clinical / management supervision . Understanding of therapy outcome measures . Comprehensive knowledge and application of therapy assessments and interventions relevant to the client group . Ability to work autonomously and as part of a team; recognising own limitations & when to ask for support and advice.
