The development of this team is in line with the Transforming Care process to support individuals with a diagnosis of IDD, who present a forensic risk and are currently detained under the Mental Health Act (1983 amended 2007).

To identify appropriate care pathways and assist with the transition from secure care / inpatient setting into the community and case management of the high-risk complex nature.

The team is passionate and dedicated to working in line with the Transforming Care Agenda making sure individual's with an ID and/or ASD don't spend longer in hospital than they have to , supporting them to a safe and meaningful life in the community .

Main duties of the job

The successful candidate will work as part of a community based multi-disciplinary Community Forensic Intellectual Developmental Disability (CFIDD) team.

The role requires an experienced nurse with specialist skills to assess patient's needs, analyse and formulate treatment plans within a multi-disciplinary framework, experience of working within forensic services or with individuals diagnosed with a Learning Disability and/or ASD.

Job responsibilities

We are looking for an experienced registered nurse (RLDN, RMN) with current registration with the nursing and midwifery Council (NMC).

The Community Forensic Intellectual Developmental Disability Service will provide specialist Forensic input in line with Transforming Care Agenda and will work alongside the Community Forensic service, serving a a population of up to 600,000.

You will work as part of a multi disciplinary team, carrying a complex community caseload and providing in reach work to secure settings to support the transition of individuals leaving hospital settings that present a forensic risk.

You will have sound knowledge and understanding of working with individuals with a recognised IDD and/or ASD , with a forensic risk. The importance of working with colleagues from multiple agencies to ensure a robust package of care is available.

You will develop and work in partnerships with IDD services, Probation, MAPPA, Courts, Police, Adult Mental Health, Private Sector and Local Authorities.

You will work directly with individuals and their families and carer groups.

You will deliver person specific interventions inclusive of training packages

You will have a sound knowledge of Mental health legislation including MHA, MCA including DoLs.

• Experience of working in or closely with a multi-disciplinary team in either IDD services or Forensic Mental Health Team.
• Previous experience with adults who have complex needs
• Demonstrate knowledge Care Programme Approach for clients in the community
• Able to demonstrate successful multi-disciplinary working
• Experience of leading clinical assessments and providing high quality feedback.

Desirable

• Demonstrate working knowledge of Mental Health Act (1983) MCA and DOLS
• Awareness of different assessment tools available i.e. HoNOS-LD, HCR20v3
• Experience of working in Forensic Services.
