The post holder will provide triage and assessment to all GP registered patients age 18+ in Walsall dealing with various mental health disorders. Cover for another locality may be required at times. The team ethos is one of working towards a recovery model of care that promotes social inclusion.

The post holder will triage referrals and undertake Trusted Assessments in accordance with the pathways relating to routine referrals.

This post holder will be working with a team that consist of a Service Manager, Clinical Lead/CBT therapist, Team Lead, Band 6 Primary Care Mental Health Nurses, Senior Mental Health Practitioners and Administrators. The team's responsibility is to provide, develop and improve services for people with non-crisis mental health conditions in primary care. The post holder will have a high level of autonomy within their role and will be expected to adhere to the parameters of the Primary Mental Health Service operational policy.

Main duties of the job

The post holder will act as Trusted Assessor undertaking Triage and Assessment, where indicated, to ensure the patient is commenced on the right care pathway to meet their needs through signposting and referral to appropriate services.

Post holders will have a high level of autonomy providing mental health triage and assessments for people with Walsall registered GP aged over 18. You will provide a service to people who present with non-crisis mental health needs. You will be expected to develop and manage all aspects of the service, such as working within the parameters of the service operational policy.

A key aspect of the role requires liaison with referrers as well as carers where appropriate and local services; understanding and utilising appropriate care pathways. Staff will be expected to work autonomously managing high volumes of referrals. Utilising their own initiative in a busy and demanding service and to be able manage change effectively.

Make complex clinical judgements, evaluating alternative courses of action and managing and advising on complex clinical issues regarding the treatment and care of patients, taking into account the opinion/advice of professional colleagues, the patient and carer, where appropriate. To seek medical guidance from the identified Responsible Clinician as appropriate.
