Do you want the opportunity to work in a blended way? The Mental Health Triage and Assessment Team may be for you.

We are looking for a band 6 specialist nurse to join our team were you will be responsible for triaging, assessing and signposting patients based on clinical need.

We are looking for a highly motivated individual to come and provide leadership to junior members of the workforce. You will be innovative, kind, compassionate and have exceptional time management skills.

Main duties of the job

  • To triage, assess and signpost patients based on clinical need following clinical intervention.
  • To support the team/clinical leads in providing clinical and supervisory leadership to junior members of the workforce.
  • To be jointly responsible with other senior colleagues for the day-to-day planning and delivery of the Mental Health Triage and Assessment Team.
  • To support the team leads and other colleagues with the implementation of policy, protocol, service/practice development and change within own area of responsibility
  • To continue to demonstrate specialist clinical skills in practice and provide support/advice re service development and the delivery of care, in partnership with service users, carers, staff, managers and other agencies

Person Specification Qualifications and knowledge Essential
  • Registered Nurse on NMC live register
  • Demonstrable experience of mentoring pre-registration students
  • Full understanding of relevant policy, legislation, drivers and their application to clinical and service area
Desirable
  • Evidence of project work that has impacted on practice and demonstrates a higher level of communication/planning/change that impacts within own area of work/responsibility
Experience Essential
  • Previous community based experience or band 6 experience transferrable to community setting
  • Evidence of promoting/supporting active user/carer involvement/participation
  • Evidence of continuing professional development (CPD) relevant to the clinical area at specialist level of care
Desirable
  • Leadership/management experience which has had a positive impact and created change within the service delivery/practice
Skills and competencies Essential
  • Effective inter-personal skills and experience in supervising and mentoring
  • Able to demonstrate effective communication skills
  • Good time management skills
Desirable
  • Ability to lead and motivate staff to embrace change
