The Team provide specialist assessment and support for people who are presenting with possible onset of Dementia and/or a diagnosis of Dementia. You will work as part of a multi-disciplinary team holistically to improve the quality of life for people with dementia, their family and carers and be committed to the concept of living well with Dementia.

The Team provide specialist assessment and support for people who are presenting with possible onset of Dementia and/or a diagnosis of Dementia.

You will work as part of a multi-disciplinary team holistically to improve the quality of life for people with dementia, their family and carers and be committed to the concept of living well with Dementia.

You will deliver a high quality clinical service to patients and their families.

You will manage a complex caseload and be responsible for clinical and caseload supervision of other team members where identified.

You will be responsible for providing the nursing process to our specialist clinical pathway.

You will deliver intensive nursing interventions, in accordance with an agreed care plan.

The service can be very fast paced and therefore the ability to work efficiently under pressure and respond to high service demand is essential. Additionally, you will need to be confident to work autonomously and organise your time effectively.

You will carry out intensive home treatment and be able to manage a complex caseload.

You will be highly motivated, have excellent communication skills and work well as part of a team.

You will havedetermination and drive committed to working holistically to improve the quality of life for people with dementia, their families and carers and demonstrate a range of therapeutic interventions and be committed to the concept of living well with dementia.

You should have experience in a supervision role and be able to make a positive contribution to the continual development of the service incorporating evidence based practice. You will be encouraged to have access to regular supervision and to maintain your professional development.
