Across Hampshire and Isle of Wight (HIOW) we have launched our plan to transform experiences in adult mental health in a programme called 'No Wrong Door'. This is a new and innovative service led by the local NHS, councils, Public Health, GPs, voluntary and community partners and people with lived experience of mental illness.

Southern Health NHS Trust's Portsmouth and South East Division and both the Swan and East Hampshire PCN's are in a position to offer these exciting opportunities for innovative, flexible, professionals who are able to network and be part of the building blocks for the transformation of frontline mental health services.

It has an extensive geography and covers 48000 patients. The GP practices are: Badgerswood and Forest, Pinehill, Riverside, The Grange surgery, Clanfield, Horndean & Rowlands Castle. We have a well-structured social prescribing team in place covering the area in three teams -- north, central and south. Also, there is a wellbeing coach in the form of a mental health practitioner for children and young people as part of that so the addition of a role for someone to help with the adult population would complete the all age offering and would allow transition from the young person's team into adults. We have a group of practices that get on well and are happy for the PCN staff to contact them, come into the surgery and generally feel part of the team.

Main duties of the job

This is a new role working collaboratively across primary and secondary care, the role will therefore be subject to change as it develops.

To work closely with secondary mental health and primary care and ensure good communication between services.

To champion a culture based on principles of recovery, choice and personalised care,

providing a role model to those who the practitioner works with.

The post holder will also provide some direct clinical care within the PCN. It will provide support to staff within the Primary Care Network and be engaged within the local community mental health team.

*To work with Primary Care stakeholders in South East area to support and help deliver the transformation work around No wrong door.

*To be able to deliver therapeutic interventions to service users whose immediate needs may need help in a primary care setting.

*To support the planning and coordination of service users' care in primary care, including those on the SMI list liaising with teams and agencies as required. To work on signposting to appropriate agencies for ongoing support.

*To discuss primary care referrals that require referral to secondary mental health care prior to making a referral

*To work closely with the secondary care mental health practitioner to provide a link between primary care services and secondary care for the Mental Health Team.

Job description Job responsibilities

*To participate in CMHT MDT meetings and MDT within a Primary care setting as required.

*Communicating service user needs and be able to assist in prioritizing the allocation of workload.

*To assess risk, develop risk management plans, review and adjust in line with the service user's needs.

*To work in collaboration with patients as experts in their own care, promoting the principles of recovery, choice and personalisation, which value and empower the service user.

*To assess, coordinate and broker the needs of services users in primary care and other relevant agencies in partnership with the service user.

*To identify, implement and evaluate, with others, areas for potential service improvement and agree further action.

*To develop, with others, measures for service standards, including benchmarking, and to identify areas for potential service improvement.

*To promote a culture in which there is a positive approach to managing risk, working closely with other senior clinicians, including medical staff, to ensure this occurs.

*To understand the range of risks potentially faced by the service user group.

*To work in partnership with others to identify and assess the nature, location and seriousness of the particular risks.

Person Specification Qualifications Essential
  • *Qualified as a registered mental health nurse, social worker, or O.T
  • *Advanced computer skills
  • *Mentorship course/ evidence of CPD
  • *Minimum of 2 years' experience of working with clients with severe and enduring mental health problems.
  • *Ability to fulfil travel requirements of the post
