Main duties of the job

  • Working in a community setting across the Cannock and Seisdon Geographic
  • Delivering a high-quality clinical service to patients and their families.
  • Care coordination of an identified caseload with access to regular and robust supervision.
  • Delivering Physiotherapy assessment and interventions within a community setting to support service users to remain at home for longer.
  • Take responsibility for the management of a caseload, which includes specialist assessment (within service specification), care planning in conjunction with service user and carer, evaluation of individual treatment plans.
  • Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.
  • Assess patient's individual care needs, develop, implement and evaluate programmes of care and rehabilitation.
  • Deliver a range of non-pharmacological interventions following specific and thorough Physical assessments to enable understanding of any expression of an unmet need and to maximise an individual's abilities.

Midlands Partnership NHS Foundation Trust has an opportunity to join our Older Adults team in South Staffordshire, you will primarily be based at St Michaels Court in Lichfield. As a Registered Physiotherapist you will be part of a team striving to achieve the best in clinical care. The service will deliver a pathway of care in accordance with the latest evidence-base, which puts the service users needs at the centre of the service.

You will be working primarily between Monday and Friday within the hours-of-service delivery 08:00-17:00.

Person Specification Essential Essential
  • Full UK Driving Licence
  • Qualified Physiotherapy Degree
  • HCPC Registered
Desirable
  • Experience in Community work
  • Experience in a range of specialities including elderly care, orthopedics, respiratory, neurology
Experience Essential
  • Experience within a broad range of specialities including but not limited to elderly care, respiratory, orthopedics, neuro, palliative care
  • Experience of working within an MDT team
Desirable
  • Experience of working in the community
  • Experience of working within the discharge to assess pathway
