We are seeking an enthusiastic Band 6 Occupational Therapist to join the Integrated Stroke Early Supported Discharge and Community Neuro Rehabilitation Service.

The Integrated service provided both stroke rehabilitation and community neuro rehabilitation supporting service users with a variety of complex needs.

This is an exciting opportunity to join an established multi-disciplinary team comprising of Occupational Therapists, Physiotherapists, Speech and Language Therapists, Psychologists, Specialist Nurses, and Neuro Rehab assistants.

This role requires the successful candidate to manage a clinical caseload across the North of Cambridgeshire. Their base will be either Peterborough, Ely, Wisbech or Doddington dependent on location nearest to home.

The role involves visiting patients in their homes, wards, or community gyms. There may be times when there is a requirement to provide clinical cover elsewhere within Cambridgeshire. The role requires agile working often using virtual consultation to manage a caseload. We will provide a smart phone and laptop to support this role.

Main duties of the job

You will provide comprehensive assessments and deliver evidence-based specialist neuro occupational therapy programmes. You will work as part of a multidisciplinary team in supporting patients with physical, cognitive, and psychosocial needs to achieve their potential.

To provide a high standard of occupational therapy. To perform advanced occupational therapy assessment of patients with neurological conditions including stroke and to deliver an individual treatment programme. To hold responsibility for own caseload and be responsible for a defined area of the service or a particular patient type, working autonomously. To work as an autonomous practitioner within the multi-professional team. Senior staff will provide support, regular formal training, clinical reasoning sessions and peer review. To be accountable for assessing, interpreting, planning, implementing and evaluating treatment to patients within professional guidelines, with the support of Advanced Practitioners. To identify patient needs, agree goals and provide appropriate therapy interventions relating to their physical, functional, cognitive and psychosocial needs. Provide appropriate equipment and rehabilitation programmes and make referrals on to other professionals and agencies as appropriate in order to meet the identified needs. To keep the patient at the centre of care, ensuring patient and carer participation in decision making. To allocate work to junior registered and non-registered staff. To provide appropriate advice to professionals from other disciplines. Provide support and education to peers, new staff, non-registered staff and students. To supervise occupational therapists, non-registered staff, occupational therapy students overseeing patient intervention and ensure that they deliver high quality patient care. To be actively involved in training, teaching and supporting junior staff, non-registered staff to achieve competencies. To be accountable for a delegated caseload held by junior registered staff and non-registered staff in the team. To provide a high standard of care. To build up and maintain key relationships with referring services and Community Neuro Rehabilitation Service (CNRS) to facilitate appropriate referrals and an understanding of the roles within the ESD team. CPFT ESD requirements: To be involved in coordinating and providing 6-month reviews for stroke survivors. To take an active role in the clinical triage duty process within the ESD service to prioritise and ensure timely response for assessments. To build up and maintain key relationships with referring services and Community Neuro Rehab Service to facilitate appropriate referrals and an understanding of the roles within the ESD team.

Person Specification Education / Qualifications Essential
  • Degree or equivalent, e.g. BSc, or Diploma entry level qualification into the Occupational Therapy profession.
  • Current HCPC registration.
  • Documented evidence of own continuing professional development.
  • Willingness to undertake Fieldwork / Practice Educator qualification
Desirable
  • Member of Royal College of Occupational Therapy
  • Member of Royal College of Occupational Therapy Specialist Section Neurological Practice.
  • Fieldwork / practice Educator qualification
Experience Essential
  • Experience in this specialist field of practice
  • Varied clinical experience
  • Experience of working in either NHS or social care
Desirable
  • Experience of equipment provision.
  • Experience of and knowledge of housing legislation, minor and major adaptations.
