Cardiac rehabilitation specialist - Physiotherapist/Nursing Permanent - 37.5 hours

We have an exciting role within our cardiac rehabilitation team, which will require a motivated individual to lead and pilot a digital cardiac rehabilitation service in order to expand our offering to patients with a goal to increase access to high quality cardiac rehabilitation.

The successful applicant will need to be a skilled clinician with experience in cardiac rehabilitation and exercise instruction. They will be expected to be able to use their skills to support the current service and deliver rehabilitation using digital services including an app-based resource and video-based platform which is being piloted until the end of the financial year 2023.

To demonstrate the effectiveness of the pilot and enable long term success the applicant will need to be confident in the collection, analysis and feedback of data and be able to report in a concise format to the cardiac project team. You will need to have excellent communication and time management skills in order to run a successful pilot with a view to longer term adoption of digital services within the organisation. You will have leadership and problem-solving skills with an interest in service development and clinical excellence.

You will need to have experience in cardiovascular disease, rehabilitation and prevention and be confident in delivering exercise to this population including complex patients.

Main duties of the job

The candidate will take an active role on the project development with support from the service leadership to operate this exciting new service.

You will complete triage, assessments and clinical work in accordance with the operation of cardiac rehabilitation promoting cardiac rehabilitation to patients.

Lead on the digital rehab programme including managing the app-based dashboard, initiating patient contacts as required.

Lead, execute and plan the video-based platform exercise classes, ensuring that a safe and effective exercise and education programme is delivered to patients.

Collect data and feedback to leadership teams agreed outcomes including to local and regional level to demonstrate the effectiveness of the digital pilot.

Support the wider team, offering support to the clinical pathway lead to support operation of the phase 3 cardiac rehabiliation service.

Job Summary

To support the delivery of the phase 3 cardiac rehabilitation service including leading the digital cardiac rehabilitation service pilot.

To provide skilled assessment, treatment and advice to adult patients in cardiac rehabilitation. Undertake a lead role in the assessment, diagnosis and formulation of cardiac rehabilitation treatment plans and service development for the digital rehab services.

To advise and train patients, carers, and families to ensure treatment is fully integrated into the patients care programme

Be the cardiac rehabilitation lead in the inter-disciplinary team for digital cardiac rehabilitation service, ensuring integration in addition to the traditional service offering, supporting patients with the use of digital services and ensuring service evaluation is undertaken and reported to the cardiac rehab group.

To supervise and teach physiotherapy/nursing assistants and technical instructors within cardiac rehab on specific topics relating to specialty of cardiac rehabilitation.

To supervise and mentor physiotherapy students.

To operationally manage and support the cardiac rehab team and represent at relevant local and regional meetings as required or directed by the clinical pathway lead.

Person Specification Essential Essential
  • Experience in cardiac rehabilitation at Band 6 or above
  • Registered with relevant professional body HCPC/NMC
  • Completion of further CPD relating to cardiac rehabiliation
  • Awareness of clinical audit and outcome reporting
  • Degree/Diploma in Physiotherapy/Nursing
  • Evidence of knowledge around cardiac rehab principles and exercise prescription
  • Completion of BACPR/ACPICR CPD training
  • Willing to undertake training to maintain competence.
  • Competent in ICT including apps and video consultations
