An exciting opportunity to help establish and clinically lead the senior mental health practitioners within the living well service in Tameside & Glossop . This newly established post will involved working with our Pennine Care NHS Foundation colleagues and our Partners in the Living well service as well as external partners to identify and support patients who are struggling with their Mental Health in the community.

This is an interesting and inspiring time to join the neighbourhood mental health team as we are developing together with our partners a service for the residents of Tameside to be able to access support for their Mental health at a time and place that is right for them.

Main duties of the job

To provide clinical leadership, support and supervision to clinical staff. and support an identified team of practitioners with advice and guidance on the management of clinical cases and hold a small case load of patients in the 12 week pathway.o To work as an integral part of the NMHT

o To analyze, validate and minimize any internal pathway blockages.

o To be an effective decision-making and clinical resource to support the Team manager in the selections and recruitment of staff, and to develop and promote strategies aimed at staff retention

o To support the Team manager to complete appraisals for relevant allocated staff within the team.

o To support the team in developing and building effective communication with all other services.

o To take appropriate action with regard to complaints and reportable incidents within the service as directed by the Team Manager.

o To provide clinical leadership and structured clinical supervision to the team.- Maintain consistency and quality of care for all serviceusers under the care of the NMHTand support effectiveand timely referral on.- Support safe ,efficient and effective high quality transitions in andout of the NHMT- Ensure the team works in a collaborative manner withinthe NMHT/LLW with other PCFT services, primary careand other partners

To be responsible for the development and maintenance of effective clinical pathways between NHMT and other services. To provide additional leadership and guidance to other teammembers in risk assessment, risk management and positive problem solving and lead on debriefs following incidents. Develop confidence and competence within the team for managing clinical cases and issues. To take a lead role in the provision of clinical leadership and structured clinical supervision to the team. To lead on and ensure that the team undertakes appropriate and indepth assessments and on-going intervention, and undertake the duties and responsibilities of a lead worker To work collaboratively with medical staff/GPs to produce clinical managementplans. To ensure the provision of the highest possible standard of care within the NMHT and ensure the team follow these principles Ensure service users within the team have an up to date integrated care record, reflective of their risk and care needs in line with policy and procedures. To assist people to achieve maximum independence through the use of care plans which are culturally sensitive, individualized, safe and consistent with accepted practice and ensure the team also follow these principles To participate and lead in mental health education and provideadvice/support as necessary to service users and their carers on the maintenance of good health and prevention of relapse and ensure the team also follow these principles To receive formal managerial supervision from the Team Manager and clinical supervision from a suitable expert. To initiate, develop and implement evidence based clinical practice. To lead on Safeguarding issues within the service and support the Senior Mental Health Practitioners where needed. To participate on carer and service user involvement, working closely with other trust leads in rolling out carer/service user support forums.Education and Training To organise the training of students, ensuring that they are provided with the necessary experiences to develop their skills by allocating them an experienced supervisor who will provide assessment, supervision and support during their allocation to the CMHT Liaise with learning partners to ensure effective learning environments. Prepare and give teaching sessions on a formal and informal basis to the team, learners and other agencies. To ensure own professional development by participating in inservice and post-basic training courses. To be responsible for creating an environment which is conductive to the training and development of all staff employed with the team, taking into account their personal objectives and learning styles, needs of the service and the benefits to their professional practice.Administration and Information Technology To ensure accurate, up to date records of visits and observations for people receiving services from NMHT and comply with team handover policies, in line with the Trusts standards for record keeping. To maintain any official documentation as required. To support and participate in the multi-disciplinary teams formal review process. To ensure that assessments and care plans produced by theteam are, in line with Trust standards.Professional Responsibilities To practice at all times in accordance with the Professional Codes of Conduct/Practice as set out by the relevant professional body.
