We are an enthusiastic and supportive multidisciplinary team offering secondary mental health services to people aged 65 and beyond and memory assessments to people of any age.
We offer a wide-range of interventions across our care group which we'd like you to help us deliver - give us a ring and we'll tell you more about what our care group offers and what you could be part of.
In exchange for your expertise we will offer you a range of opportunities to grow and develop, including: regular management supervision, profession specific supervision, further education, coaching and mentoring.
Main duties of the job
Arranging, contributing to and documenting care reviews, including telephone, video and face to face reviews - as appropriate and defined within the care group SOP. To work within the care programme approach.
To undertake holistic assessments of people's mental health and physical health needs.
To undertake occupational therapy assessments to support memory assessment
To develop holistic care plans and work collaboratively with clients to ensure that their needs are addressed.
To identify and report any safeguarding issues as they may arise.
To administer medications in the community and to review medication efficacy
To supervise junior staff members and to support students in placement
To work collaboratively with the multidisciplinary team as well as patients, carers, family members and internal and external agencies.

Person Specification Qualifications Essential
- Qualification: RMN, RLDN or OT
- Achieved Grade beyond a pass
- Minimum of 1 years experience - or less than a year consider interview for B5
- More than 2 years experience working in mental health care
