The North Tees Community Team provides a specialist service to adults with a learning disability who live in the Stockton and Hartlepool locality. The combined psychiatry caseload for the two localities is 120, with between 0 and 2 referrals per month. It is anticipated that the post holder will hold a psychiatry caseload of around 40-50 patients from the Hartlepool locality. The majority will be jointly managed with the wider MDT. The initial timetable for the post contains two outpatient clinics per week, with additional domiciliary visits as required. Given the caseload numbers outlined above, the majority of clinic time will be spent reviewing existing cases.

The Trust is reviewing its response to NHS Englands position statement on the Care Programme Approach. At the time of writing, the Trust continues to support service users with more complex needs under care coordination. Consultant psychiatrists are not expected to act as care coordinators.

The post holder will be based at the Centre for Integrated Living in Hartlepool, where the team is co-located with the Hartlepool Learning Disability Social Work Team. Office space and clinic space will be provided on this site. Some meetings will also take place at the teams other base at Wessex House, Stockton.

Main duties of the job

To assess, diagnose and manage patients with mental illness, autism and challenging behaviour

Ensuring effective holistic multidisciplinary assessment and care planning with service users, carers and the multidisciplinary team

Planning of all treatment needs including prescription of medication

Ensuring the physical health needs of patients are met

Management of risk, and weighing complex risk information to deliver care that is in the best interests of the recovery of the individual. Supporting the MDT in this by clear communication, containing anxiety, and maintain hope for the patient.

Work with medical and nursing colleagues to ensure medical input to daily patient flow process to help patients in different parts of the system access care appropriately

Maintaining a focus on outcomes, both in terms of clinical outcomes and patient experience.

Person Specification Qualifications Essential
  • MB BS or equivalent medical qualification.
  • Fully registered with the GMC with a licence to practise at the time of appointment.
  • Eligible for inclusion in the Specialist Register or within 6 months of gaining CCT
  • Approved clinician status OR able to achieve within 3 months of appointment
  • Approved under S12 OR able to achieve with 3 months of appointment
  • Qualification or higher degree in medical education, clinical research or management.
  • MRCPsych
Clinical Knowledge & Skills Essential
  • Excellent knowledge in specialty.
  • Excellent clinical skills using bio-psycho-social perspective and wide medical knowledge.
  • Able to manage clinical complexity and uncertainty.
  • Makes decisions based on evidence and experience including the contribution of others.
Teaching & Training Essential
  • Able to deliver undergraduate or postgraduate teaching and training.
  • Evidence of achievement in education, research, audit and service improvement: awards, prizes, presentations and publications.
