We are looking for a passionate, enthusiastic, self-motivated, and flexible nurse who is looking to take the next step in their nursing career and join a well-established and experienced multidisciplinary team within Merton Primary Care Recovery Service, which is part of Merton Uplift.

Merton Uplift provide primary mental health care services for people aged 18+ who are residents in the borough and/or registered with a Merton GP. It comprises three service streams; a Wellbeing Service delivered in partnership with third sector partners, an IAPT service delivering a variety of psychological therapies and a primary care mental health service. Merton Uplift, whilst offering a wide breadth of services, maintains a focus on promoting wellbeing and independence. We also work in adject with the Primary Care Network, where we have Mental Health Practitioners embedded within GP surgeries.

You will work to provide a high-quality nursing care for your service users, targeting and prioritising people with mild to severe mental health needs. You will manage a caseload of clients with complex needs, and use evidence based/client centred principles to assess, plan, implement and evaluate interventions in conjunction with the MDT, working closely and collaborating effectively with all agencies and partner organisations involved in the care of our clients.

Main duties of the job

You will be working as a Registered Mental Health Nurse, with experience of working with adults with mental illness, possess good communication skills; be able to lead, motivate and develop staff and manage difficult situations, and be able to cope with stress and change. You will also have a sound knowledge of clinical issues within mental health services, and you must have a valid UK driving license and access to your own car. You will be expected to use your initiative and will be required to work independently when visiting and assessing clients, carers, and relatives. You will need to be able to administer, supervise and monitor the therapeutic effects and side-effects of prescribed medication, providing clear advice and information in accordance with the Trust Policy and the NMC Standards for administration of drugs.

You will be expected to hold an ENB 998, Mentorship equivalent qualification and adhere to the NMC standards. You will also be expected to maintain your registration through Revalidation and continuous professional development.There are training opportunities and development through the Trust and external Clinical Practice Development courses.

To offer comprehensive triage and initial assessments, where needed, in line with agree health and social care eligibility criteria to service users referred to the service. To be responsible, under supervision, for the co-ordination and transfer to the relevant treatment team of a designated caseload of services users with enduring mental health problems or service users who meet the threshold for primary care support. To maintain high standard of individualised client care, recognising factors and events that are likely to affect the mental, emotional and physical wellbring of clinets/carers and provide advise and support when required. To develop and maintain a high standard of practice, based on assessment, through the use of a range of psychosocial approaches, within the health and psycho-social framework. To communicate/liaise effectively within relevant agencies to ensure that an integrated program of care is provided throughout. To implement and/or participate in crisis intervention measures with the client/family or in conjunction with other agencies. To administer, supervise and monitor the therapeutic effects and side-effects of prescribed medication providing the client with clear advice and information in accordance with the Trust Policy and the NMC Standards for administration of drugs. To organised and participate in the admission, discharge and transfer of clients in accordance with Trust Policies. To provide clients and carers with verbal and written up to date information on the services offered by the Trust, how to express their views, complaints procedure and what they can expect from the CMHT. To attend daily clinical meetings to provide the team with relevant information inrelation to caseloads, referrals and to feedback on newassessments. To develop and maintain effective working relationships with in-patient facilities regarding admission and discharge of clients. To facilitate carers assessments, provide the appropriate care plan to meet identified needs and to monitor efficacy. To ensure that risk and Safeguarding issues are fully assessed, addressed and documented. To maintain accurate clinical documentation stated in Trust guidelines.

Person Specification Education and Qualification Essential

• oRMN (current NMC registration) or HCPC registration
• oDegree in Nursing/OT

Desirable

• oDiploma/BSc in Community Nursing
• oEvidence of post registration training related to the provision of relevant psychosocial interventions.

Experience Essential

• oDemonstrable experience of working with people who experience serious ongoing mental health problems in a variety of settings
• oExperience of working in a multidisciplinary team
• oExperience of assessing and managing crisis with those with a severe mental illness

• oExperience of working community nursing
