We are excited to be offering a position in our Intensive Support Team for a B6 CPA Coordinator.

The Intensive Support Team works with adults who have a learning disability and who present with high levels of risk. This may be due to behaviours that challenge and/or mental ill health. Our overarching aim is to prevent admissions to hospital under the Mental Health Act - in line with Transforming Care Programme.

The post holder will be required to follow the Trust's CPA Framework in assessing and planning care for our service user group.

Main duties of the job

The post holder will work collaboratively with other professionals and agencies to provide a high quality, comprehensive and dynamic specialist clinical input with the central aim of maintaining and improving the mental health and well being of adults with learning disabilities through a CPA framework.

This will include working directly with service users (and their families) who have mental health needs and/ or behaviours which require intensive assessment and support, both in the community and other settings.

We are a friendly and dynamic team, comprising of CPA Coordinators (LD Nurses/OTs), HCSW and DSR/CTR Coordinator. We enjoy good links and access to the wider MDT in a timely fashion, so as to meet the needs of our service users.

We sit within the wider Learning Disability Service based at Waddiloves Health Centre (LD Nurses, HCSWs, Psychologists, OTs, Speech and Language Therapists, Physiotherapists, Psychiatry and LD Clinics)
