Are you a highly skilled clinician with a passion for helping people achieve a successful transition from inpatient care to their next step? We are seeking a talented Discharge Manager to join our Assertive Outreach team and support service users in their journey towards a better quality of life.
Main duties of the job
Responsibilities:
In this dynamic role, you will be responsible for collaborating with the clinical team to identify appropriate placements and support the completion of relevant assessments and referrals. You will also present funding packages to panels when necessary, working closely with local teams and private partners to ensure that service users who access acute and out-of-area beds receive the support they need for a successful discharge.
As a Discharge Manager, you will play a key role in supporting service users, carers, and clinical teams in the development of comprehensive discharge plans. You will work independently with the AOT teams and private partners to implement robust and supportive discharge packages. You will also coordinate and chair the AOT and wider bed management review function to improve bed flow within assertive outreach.
Qualifications:
We are looking for a candidate with exceptional clinical skills, including the ability to work independently, undertake comprehensive assessments, and develop robust discharge plans. Comprehensive IT skills are essential, as there will be a requirement to provide comprehensive performance data regarding clinical activity.

Person Specification Qualifications Essential
- Nursing or AHP registered
- Masters degree in health related subject
- Communicate effectively at all levels across the organisation
- Evidence of expertise in clinical skills
- Ability to work under pressure and meet deadlines
- Experience of assessing and providing care to people with complex needs
- Extensive understanding of CCG processes in respect of funding
- Clinical management in more than one field, e/g/ acute community, primary care
