We are excited to announce a new role based in the London Borough of Merton with a focus on supporting young people to transition from Child and Adolescent Mental Health Services (CAMHS) to Adult Services. The post holder will also support families and work alongside teams and liaise with wider organisations ensuring wider system collaboration to wrap around support for our young people.

These new innovative roles have been developed to ensure our young people moving from CAMHS services into Adult services are sufficiently prepared and supported to navigate adult provision and networks, whilst maintaining their progress and promoting independence.

The aim of the CAMHS Transition posts is to support the successful transition of young people aged 17-25 into appropriate mental health services and to enhance the social and mental health outcomes of young people experiencing difficulties as they grow into adulthood.

Main duties of the job

It is acknowledged that the transition from child to adult services creates risks for young people who may fall through the systems due to not having the right support. This can result in poor mental health and social outcomes, leading to loss of human potential and increased burden on health and social care services over a lifespan.

This post will directly inform and update Transition Protocol between CAMHS and AMHS based upon the latest evidence of best practice. The post holder will also be responsible for supporting teams and provide training, where necessary, to educate and skill up professionals and stakeholders in the needs of those young people moving from CAMHS into adult services. Core to the role with be ensuring excellent collaboration between the young person and families to support the young person's journey across services.

We are looking for an individual who has a strong track record in system wide working and communication across teams, who demonstrates excellent leadership qualities and someone who is inspirational and engaging with young people, staff and external partners.

The Transition worker posts are positioned within Tier 3 CAMHS teams. Tier 3 CAMHS is an CYP-IAPT service which provides targeted/goal focussed, evidence-based interventions to children, young people and their families up to the age of 18. CAMHS receives referrals for children and young people who have moderate to severe/complex mental health needs. Often a lower level/Tier 2 interventions may have already been utilised. Referrals are assessed initially by the Single Point of Access or may have come from the CAMHS Emergency Care Service (CECS). It is a multi-disciplinary team and clinicians are keen to ensure good outcomes for the young people within the borough.

  • Enhance awareness of transition issues, challenges and services available to young people
  • Foster excellent working relationships between Children and Adolescent and Adult Mental Health services.
  • Join a multi-disciplinary team at one of five Tier 3 CAMHS locations
  • Promote mental health wellbeing and physical outcome
  • Promote stability in education and work
  • Reduce episodes of crisis
  • Agreed job plan with specified activity and throughput levels
  • Co-ordinate care for children, young people and families on their caseload who are in process of transition
  • Manage risk within team, Directorate and Trust structures
  • Work to relevant professional and ethical guidelines, and within the framework of Directorate and Trust policies and procedures
  • Contribute to the review and update of protocols for transition between CAMHS and AMHS services
  • Contribute to screening and outcome measures in clinically meaningful ways to demonstrate treatment effectiveness and seek feedback about user experiences


Person Specification Training & Qualification Essential
  • First degree in mental health nursing
  • Professional registration with NMC
  • Evidence of relevant professional development
Desirable
  • Further post registration training in child and adolescent mental health
  • An appropriate professional qualification to supervise students in training. (e.g. ENB 998 or Practice Teacher Award)
  • A postgraduate qualification in a therapeutic modality relevant to child mental health
Experience Essential
  • Significant post qualification experience
  • Significant experience of working with self-harm
  • Significant experience of working with adolescents with mental health difficulties
  • Clinical risk assessment and management of vulnerable client groups
  • Experience of developing services and influencing and managing a change process
  • Establishing collaborative relationships across professional groups and organisations and managing those relationships with sensitivity and diplomacy
Desirable
  • Experience of Racial Awareness/Diversity Training
  • Conducting service evaluations
  • Working collaboratively with service users and/or carers to develop services
Skills & Knowledge Essential
  • Knowledge of current child mental health guidance and legislation
  • Ability to co-ordinate and deliver a day programme
  • Ability to plan and develop local policy and procedure
  • Ability to plan and develop local policy and procedure
  • Demonstrate cultural awareness and sensitivity to minority and ethnic groups
Desirable
  • Database skills
