An exciting opportunity has arisen for an Occupational Therapist to lead in the Gospel Oak Camden Integrated Community Health Service (CICH).CICH celebrates three District Nursing and Therapies teams, working together in a unique integrated model to meet and address the health needs of the local community .

The post holder will work jointly with the Band 8A Nurse Lead to be responsible for operational management and leadership of the Gospel Oak community team ensuring the coordination of the care for patients on the team caseload, as well as the appropriate deployment of staff to meet the needs of these patients. The post holder will also ensure that the team provides effective assessment, planning, implementation and evaluation of care that meets professional standards.

The post holder is clinically responsible for managing and leading the inter-disciplinary rehabilitation OT, Physiotherapy, Clinical Psychology team at Gospel Oak.

Main duties of the job

With a specialist qualification in Occupational Therapy the post holder will also have a wider role across CICH to support with specialist clinical supervision and lead on projects where Occupational Therapy specialist practice input or professional leadership is specifically required. The post holder will also work jointly alongside Physiotherapy Leads across Camden Integrated Community Health Care (CICH) for the falls pathway in the borough. The invaluable contribution of OT and Physio has been recognised and our service is integral to the successful delivery of this service. This means that the post holder will need to apply enhanced knowledge of falls management and be confident when representing the service at multiple different forums/workshops and project meetings. There is a strong emphasis on new ways of working with the developing multi- disciplinary neighbourhood working and the post holder will be required to act as clinical lead as an Occupational Therapist to enhance multi-agency collaborative working in the neighbourhood.

role model in expert practice facilitating and supporting the delivery of effective rehabilitation programmes by use of advanced clinical judgement and decision-making skills.1.8 Ensure quality standards in holistic service provision including health promotion and education in response to the changing health needs of the local community.10.9 Overseeing and support staff to develop robust risk management plans that support individuals independence and daily living within their home setting.10.10 Secure appropriate expertise to provide specialist support to ensure that patients are referred to other practitioners when required and that appropriate supplies and equipment are in place and reviewed as per local policy.10:11 Work within the organisations framework of informed consent10:12 Ensure that professional competency frameworks for therapy staff and students is robust and supportive to develop specialist interventions within the integrated teams.10:13 To be conversant and have a range of in depth knowledge of the Trusts policies and procedures and to utilise these fairly.10:14 Manage the budget and allocation of resources appropriately alongside the District Nursing Lead.2: OT Professional Leadership2.1 To attend and represent CICH at equipment contract meetings and oversee specialist OT sign off approval for equipment.2.2 To support and lead on all OT clinical development work across the CRTs2.3 To provide leadership and support with all OT recruitment in other teams, including interview, formulation of questions, scenarios as there are no other OT leaders across CRTs2.4 To support the induction, training, appraisals and probationary reviews of all new B7 OTs2.5 With expertise in Occupational Therapy the post holder will provide clinical supervision for the B7 OTs across three CICH Services2.6 Be a professional point of contact for OT support and advice for other community teams.2.7 To support HCPC and professional practice queries related to OT staff across the named teams2: Collaborative and Partnership Working2.1 The post holder will be a champion of integrated working to ensure joined up seamless patient care in the community through complex partnership working across multiagency teams.2.2 To attend any Occupational Therapy (OT) leaders forums and dissemiatee, trust, national, regional and integrated community service level information back to the OTs2.3 To represent the role of OT at neighbourhood hubs as part of specialist multi-agency working2.4 To facilitate and participate in Trust-wide groups and partner agency forums as required to establish, develop and maintain collaborative and productive integrated partnerships with individuals and agencies working in the field of the community health care.2.5 Be responsible in delivering strategic improvement in the care provided to patients based on up to date evidence based policy and practise.3: Facilitating and Teaching to enhance quality practice3.1 The post holder will provide specialist education and training to staff within the team to improve clinical performance.3.2 Provide specialist OT teaching and supervision to students and new staff as required.3.3 Ensure that Occupational Therapy fair share student placements are being met and that junior staff have educator training.3.4 Support the Band 6 OT Camden rotations4: Communication4.1 The individual will demonstrate a high standard of electronic record keeping and documentation with adherence to CNWL NHS Foundation Trusts policy.4.2 The post holder will have an in depth understanding of professional guidelines and the application of confidentiality4.3 The post holder will work at the interface between the team and statutory and voluntary organisations, to facilitate cohesive approaches to meeting the diverse needs of people in Camden.4.5 The post holder will maintain efficient and effective communication both, verbal and written with clients, their relatives, colleagues and external stakeholders.4.6 To comply with current data collation and protection systems, provide written reports as requested and complete statistical returns in accordance with CNWL policy requirements.
