An exciting opportunity has arisen for experienced practitioners with criminal justice interest, to work in Staffordshire's Integrated Offender Health (IOH) Service. Staff would be expected to work in the community within the Outreach element of the team. This will include providing high quality assessments of people attending and being released from Police Custody and supporting people under the care of the Outreach Team. The service will be delivered to people of all ages including those under the age of 18yrs who present with any vulnerability including but not limited to; mental health, physical health, learning disability, head injury, unemployment, housing, educational needs etc. Within this role there will also be an opportunity to work within Police Custody at Watling Street.

This exciting service is currently funded by NHS England and is delivered by Midlands Partnership NHS Foundation Trust.

The successful applicants must be experienced in risk assessment, highly motivated and someone who is looking for a new challenge and have the skills to make a difference.

Due to the nature of the work posts may be subject to an Enhanced Criminal Records Bureau disclosure check (police check), through the Criminal Records Bureau as stipulated under part V of the Police Act 1997. Some posts may also require enhanced Police Vetting.

Job description Job responsibilities

The Post holder will be expected to work across all the community settings in the Staffordshire area on a daily basis.

The Post holder will be expected to conduct assessments within the community providing relevant information to partner agencies, Court & Probation services to support people in custody and offer diversion options from the criminal justice system to more appropriate services where needs are identified.

The post holder will be required to work autonomously within this role and be accountable for managing referrals; prioritizing referrals and ensuring clinical and electronic record keeping standards are adhered too.

There may also be times where the post holder would be required to work, as directed by demand and need, across the wider service including Court and Custody teams

The service is for both male and female service users and as the Black Country area consists of a diverse population, staff would need to be mindful and skilled to work with people of different ethnicities and backgrounds. They must also be able to demonstrate experience of working with both adults and children/youths.
  • Working in an Integrated Offender Health Service in Staffordshire, dependent on skill and service need.
  • Delivering a high quality clinical service to patients and their families.
  • Managing complex assessment in court, custody suite and Outreach Service. Responsible for clinical and caseload supervision of other team members where identified.
  • Diverting and Liaising with multiple criminal police and other partners.
  • To provide written reports for Crown or Magistrates Courts or to attend and give verbal evidence as required. To offer considered recommendations to these bodies.
  • Responsible for participating in the setting of quality standards, including the auditing, monitoring and reviewing of practice in line with current clinical guidance practice and policy.
  • Assess patient's individual care needs, agree and refer on to appropriate agency.
  • Complete and contribute, where appropriate to robust risk assessments and risk management plans. Deliver and promote positive risk taking, where appropriate, to improve quality of life and maximise occupational performance/independence.
  • Maintain an effective reporting system by observing and reporting verbally and in writing on patient condition
  • Utilise agreed outcome measures to review the effectiveness of process, as part of Liaison and Diversion Service.
  • Participate in the development of the service to ensure clinical governance processes are actively implemented at a team level.
  • Assist in ensuring that the aims and objectives of the service (as set by the Team Leader) are fulfilled and to identify factors which may inhibit these from being achieved.
  • The post holder will be required to work within the HCPC Code of Practice and professional guidelines.
  • Maintain an up-to-date level of professional competence within the specific environment.

Assist in the implementation of evidence-based nursing practice and participate in research and other projects as required

  • Registered mental health Nurse on NMC Register
  • Post-registration experience, evidence of formal courses and training at diploma/degree level.
  • Registered Mentor
  • Registered with a professional body (OT, SALT, SW)
Desirable
  • Leadership qualification
  • BSc degree in nursing or equivalent level
  • Experience of working in community setting and making decisions autonomously
  • Leadership qualification / existing accreditation relating to Practice Educator / Work Based Supervisor / AMHP
