Joint Emergency Team Woolwich

The Joint Emergency Team aims to prevent inappropriate hospital admissions and facilitate early discharges from hospital by acting as the gateway to the Integrated Community Services within the borough of Greenwich. The team responds to referrals requiring intervention within 2 hours which includes the falls pick up service.

The team provides a multi-professional assessment of health and social care needs to support the immediate crisis, identifying specialist assessment and rehabilitation requirements. The team is able to monitor and provide intervention to support patients through an acute period of illness. The team is able to provide equipment necessary to maintain patients / clients at home.

JET is a seven day service that operates from 08:00 to 20:30, including bank holidays.

Main duties of the job

The Occupational Therapist will work as part of a multidisciplinary team, including health and social care colleagues to meet the needs of clients referred to the Joint emergency Team, ensuring the highest possible standards of practice.

• To conduct specialist and generic assessments with the MDT for clients referred to Joint Emergency Team
• To organise transfer to intermediate care facilities (at home or intermediate care beds) as appropriate, care packages as required and onward referrals.
• To provide advanced specialist Occupational Therapy input to clients referred to the team including patients on the virtual ward.
• To provide clinical leadership into the day-to-day delivery of Occupational Therapy in the Joint Emergency Team, ensuring the highest possible standards of practice. This includes supervision and support of junior staff and students.
• To support the Team Manager in establishing and monitoring a robust clinical governance framework for the Joint Emergency Team.
• To work in line with the Trust objectives for community practice.

Job description Job responsibilities

• Be able to work alone in the community in a variety of settings, independent of multi- disciplinary support whilst in direct contact with clients and their carers
• Be professionally and legally responsible and accountable for all professional activities including occupational therapy assessment, diagnosis and treatment of clients referred to the team as an autonomous practitioner and in partnership with the client/carer; obtaining consent in accordance with professional guidelines and local team
• Demonstrate and apply theoretical knowledge of acute, multiple, complex and long-term pathologies and impairments in the assessment and management of
• Use a range of highly specialist clinical assessments and skills to ensure effective management of clients
• Recommend appropriate interventions to promote independence and safety, utilising effective clinical reasoning skills and evidence-based practice
• Carry out clinical risk assessment and complete risk management plans relevant to the client, including risk assessments for equipment provided. Ensure management of clinical risk identified.
• Complete an individual care plan for clients who require input from integrated community services (bed-based or home based input).
• Engage in joint patient assessment/ treatment with more junior staff as need dictates.
• Work in a co-ordinated manner, communicating effectively with other team members and the wider clinical network to ensure that clients needs are met.
• Provide specialist advice and education to client, carers and relatives based on clinical evidence and identified needs.
• Provide highly specialist advice & second opinions to more junior colleagues/others regarding the management and care of patients/clients physical needs.
• Maintain up-to-date clinical written records and electronic database in accordance with local and professional standards; and also writing comprehensive reports on clients including discharge letters to medical referrers,
• Ensure safe, appropriate and effective provision, installation and demonstration of equipment.
• Utilise and further develop knowledge of minor/major adaptation provision and non-stock equipment availability and use.
• Participate in MDT meetings and family conferences as required.
• Seek advice/guidance from managers or more experienced colleagues when required.
• Be able to identify when and how to terminate involvement or refer onwards to appropriate teams/services and complete input in a timely, seamless manner.

Person Specification Qualifications Essential

• Recognised Occupational Therapy Degree Qualification or equivalent
• UK Health and Care Professions Council (HCPC) Registration

Experience Essential

• Minimum of 6 years post registration experience
• Evidence of recent Continuing Professional and Personal Development
• Wide range of experience in different setting which includes community

Knowledge Skills and Abilities Essential

• Experience of managing change
• Knowledge and understanding of safeguarding adults and children
• Experience of undertaking supervision and personal development plans
