To be personally responsible and professionally accountable for the assessment, care and onward referral of service users who have severe mental health problems (including dual diagnosis) who are homeless or at risk of homelessness.

To ensure effective liaison and joint working with all agencies involved in the care of the homeless with mental health problems.

To assist this client group in appropriate access to health services.

To work in a flexible way to provide an easily accessible Community Mental Health service to the homeless.

To actively engage homeless people with mental health services, completing assessments to determine the most appropriate care required and onward referral to services where appropriate, including voluntary agencies, social care and drug / alcohol services.

To manage a caseload of service users who are undergoing assessment, waiting for care from other services or do not meet the criteria for other mental health services.

To complete comprehensive risk assessments and management plans on all service users in line with Trust policy.

To complete Payment by Results (PbR) clustering in line with Trust policy.

Main duties of the job

To be actively involved in the discharge planning and follow up arrangements for homeless people in the BCP area, from MH in-patient units, including providing the 7-day follow-up visit for service users who do not require on-going care from the CMHT.

To fully implement and adhere to the Trust policies for CPA, Care Management and risk management.

Administration of medication, ensuring compliance with nursing responsibilities under the Medicines Act (1971) Trust policies and clinical procedures (dependant on profession of the post holder)

Ensure compliance with the Mental Health Act 1983 (including Community Treatment Orders).

To provide cross-cover for colleagues in the Mental Health Homeless Service across Dorset and AOT colleagues where required.

To participate and contribute to Team meetings and Homeless HealthCare Network Group.

Bringing any ideas for initiatives/innovations to improve services to the attention of the Team Leader/Service Manager, and if agreed to assist in implementing.

Person Specification KNOWLEDGE, SKILLS AND TRAINING Essential
  • Registered practitioner to degree / diploma level band 6
  • Membership of the relevant Professional Body
  • Ability to work autonomously in the community making appropriate clinical decisions and managing risk without constant and direct supervision. Able to identify when issues require joint working and escalation into the wider multi-disciplinary team
  • Knowledge of lone working policy requirements and the risk assessment skills required to promote safe lone working.
  • Ability to manage own workload in respect of a designated caseload, appropriately prioritising time and responding to clinical needs, being flexible and able to reprioritise as situations rise
JOB SPECIFIC EXPERIENCE Essential
  • Experience of multi-disciplinary working
  • Ability to teach staff new to the clinical area, less experienced staff, students and members of the multi-disciplinary team
  • Ability to work autonomously in the community making appropriate clinical decisions and managing risk without constant and direct supervision. Able to identify when issues require joint working and escalation into the wider multi-disciplinary team
  • Knowledge of lone working policy requirements and the risk assessment skills required to promote safe lone working.
  • Ability to manage own workload in respect of a designated caseload, appropriately prioritising time and responding to clinical needs, being flexible and able to reprioritise as situations rise
PERSONAL QUALITIES/ATTRIBUTES Essential
  • Ability to use own initiative within sphere of authority
  • Willingness to advance own clinical knowledge, skill and competence based on current evidence
  • Able to analyse and assess sometimes challenging and complex situations requiring interpretation of potentially conflicting situations with the ability to make judgements on appropriate courses of action
  • Evidence of demonstrating the Trust's values and behaviours. Ability to be reflective and recognise the value of supervision to manage emotional demands of clinical role.
