Responsible for the specialist assessment and care coordination of clients and provide relevant appropriate treatments when appropriate to allow them to maintain independence for as long as possible in the community.

Main duties of the job

An exciting opportunity has arisen for a practitioner to join an Older Person's Community Mental Health Team.

Person Specification Qualifications and Knowledge Essential
  • Significant post regestration experience within Old Age psychiatry
  • Registered professional
  • Skilled in risk assessment and risk management
  • Understanding of the Mental Health Measure Wales
  • Knowledge of evidence based interventions
  • Up to date knowledge of assessment processes
  • Awareness of clinical governance
  • Understanding of contemporary approaches / developments in mental health
  • Awareness of appropriate professional guidelines for practice
Desirable
  • Educated to degree level or equivalant
  • Post qualification training in relevant areas
  • Understanding of the CHC process
  • Mentorship training
Experience Essential
  • Able to prioritise and manage own caseload
  • Evidence of refelective practice
  • Experience of multi-disciplinary team work
  • Committed to professional development
Desirable
  • Hospital community mental health experience with older people
Aptitude and abilities Essential
  • Knowledgeable and skilled in assessment requiring the ability to organise, plan and signpost care
  • Flexible to meet the needs of the service
  • Proven clinical skills commensurate with the post
