This is an exciting position which will focus on the physical wellbeing of our service users with mental health needs. The post holder will be responsible for the leading on the physical health care agenda within Trafford Community Mental Health Teams under the supervision of the North CMHT Manager
The post holder will ensure the delivery of all physical health care CQUIN requirements and trust key performance indicators. They will act as a specialist resource and advisor to care coordinators within the community teams. The post holder will work across community settings, including clinic bases and home visits.
The post holder will be required to work, for the majority of time, as an autonomous clinical practitioner for patient groups across a range of clinical settings,. The post holder will be expected to forge strong links with the Care Coordinators, Physical Health Support staff, Primary Care services particularly GP practices.
The post holder will be responsible for efficient running of the weekly Clozapine Clinic held at The Moorside Unit.
The post holder will be responsible for the clinical supervision of a small team of physical healthcare practitioners.
Main duties of the job
Management of Physical health support staff.
Assessment of Physical healthcare and developing management plans
Experience of completed Physical Health investigations and associated interventions
Experience of undertaking physical health observations
Effective communication skills and experience in the management of others
Ability to act on own initiative and provide advice and support to nursing staff
Ability to liaise and communicate with a range of internal and external personnel
Ability to use clinical information/IT systems
To implement change within a clinical setting

Person Specification Education and Training Essential
- Registered Mental Health Nurse
- Evidence of post registration qualifications or training
- Training in: venepuncture, cannualtion IV Therapy, ILS, Ecg reading and taking, reading and sign posting blood results
- Acute Chronic wound management, clinical examination skills, Mentorship in practice. Willingness to undertake the above
- Substantial experience of working as an autonomous clinical practitioner
- Experience of working with mental health patients in any health/social care setting
- Experience of working as part of a team in order to achieve a shared vision/goal
- Experience in more than one specialism, Leading change in a health care setting,Experience of providing support to vulnerable/social excluded individuals/groups
