An opportunity has arisen for a suitably qualified and experienced Learning Disability Nurse or Mental Health Nurse to work as an AWP employee within the Community Learning Disability services provided in Bristol by Sirona care and health .

You will expect to be clinically supervised by a senior Clinician or Senior Nurse from Sirona care and health and will be line managed and supervised by the team manager of AWP Forensic Community Learning Disability team.

You will be one of three nursing practitioners who support the needs of people with Learning Disabilities who have additional mental health needs.

The role involves CPA co-ordination, holding a caseload of clients who may have complex presentations, providing consultation and mental health advice to members of the wider Multi disciplinary team with in the Community Learning Disability service and mainstream services.

Main duties of the job

Working in a collaborative, pro-active and preventative way to reduce the need for admission to hospital and deliver interventions to promote recovery and wellness. You will work closely with Consultant psychiatrists to assess and monitor the mental health of service users, monitor the effects of medication and provide therapeutic interventions .

Working mainly across the Bristol area and will work with service users in a variety of settings. Occasional travel to further locations may be required to attend CPA / CTR meetings and support the timely discharge of service users in out of area hospital placements in accordance with the Transforming Care agenda.

You will have the ability to understand how mental illness in people with Learning Disabilities may be different from a mainstream presentation. The ability to implement reasonable adjustments in the assessment, planning and delivery of mental health care is an essential part of this role.

Liaison with mainstream inpatient services about what unique challenges someone with a Learning Disability might face in hospital is an important aspect of this role. You will support service users where possible to access mainstream services and will provide advice and consultation to these services.

Person Specification Essential Essential
  • Current relevant registration with the NMC- registered mental health Nurse or registered learning Disability Nurse
  • Demonstrates substantial post-registration experience gained working in a range of front-line adult mental health services, undertaking assessment and delivering interventions
  • Demonstrates substantial experience of assessing risk and developing risk management strategies
  • Able to articulate a detailed understanding of the relevant legal frameworks/legislation including CPA process, Mental Health Act , Mental Capacity Act, and Safeguarding.
  • Highly developed verbal communication skills, able to engage effectively with people at all levels even when a more assertive approach is needed.
