We are looking for an experienced mental health nurse to work within the Offender Healthcare Service at HMP Wandsworth . This is a unique opportunity to be part of Oxleas NHS Foundation Trust and to work at one of the largest prisons in the UK. You will assist us to provide excellent mental health care to our population.

Offenders are a challenging population group to work with and they often have complex healthcare needs. Offenders often have chaotic lives in the community and a history of poor engagement with community healthcare services. With high rates of substance misuse and mental health problems, this is a real chance to make a difference

To help us deliver our extensive range of 24-hour on-site services, you'll be a qualified registered mental health nurse (RMN) with a passion to deliver high-quality clinical care. You'll be self-motivated, have a sound knowledge base and excellent communications skills. Experience of working within the prison environment is preferable but not mandatory as full induction and training will be provided.

Main duties of the job

• To provide quality healthcare to offenders in HMP Wandsworth
• To provide safe and effective care pathways for all offenders identified for transfer to external NHS or independent in-patient services
• Working alongside the responsible prison clinical team, the post-holder will act as the link between external providers and the prison teams to ensure a co-ordinated approach to transfer planning, which results in effective and timely transfers in accordance with national time scales (14 days)
• Maintain accurate and timely transfer activity, information and data on the electronic data tracker system
• Be responsible for timely referrals to external providers
• Be responsible for tracking the progress of all referrals
• Promote timely medical and nursing assessments by external providers
• Maintain close links with the Ministry of Justice to ensure timely transfers
• Throughout the duration of an offenders stay in external services the post holder will act as a point of contact to ensure the smooth flow and exchange of information and attend all relevant forums relating to the offender

Job description Job responsibilities

• To case manage in conjunction with other practitioners, offenders admitted into hospital (secondary care) presenting with acute / long term physical health concerns.
• Be fully conversant with Trust policies, CPA, and the Mental Health Act
• Provision of high quality assessments and reports/ updates regarding patients in partnership with Lead GP and Psychiatrist.
• Maintain oversight of the escort and discharge database for each site.
• Work in partnership with Service Manager to develop mechanism that establishes effectiveness of Transfer and discharge post through evidence of cost savings or improved patient pathway.
• Work in partnership with General Practitioners to ensure timely and appropriate referral process/pathway in line with sentence status of the patient.
• Support the development of prison based care to minimise length of hospital admission and promote site based interventions.
• Develop robust evidence based protocols for the safe admission back to the prison upon discharge from hospital.
• The post holder will be expected to teach health professionals and criminal justice staff on prison transfer related issues, disseminating learning, developments and examples of good practice
• Act as the first line of arbitration for disputed transfers This will involve identifying alternative solutions within national and local policy and protocol.
• Report all disputes and delays to site Head of Healthcare and Service Manager in a timely fashion
• Ensure that appropriate systems are in place, which enable and facilitate the effective Care Programme Approach, care coordination and continuity of care
• Develop and maintain effective channels of communication, professional links and interfaces with NHS and independent providers, statutory agencies, the third sector and the Prison Service.
• Working closely with these agencies, provide person centred and integrated packages of care, and co-ordinate activities to achieve joint working and integrated care arrangements.
• Actively promote the development of multi-disciplinary clinical practice, embedding as appropriate current research and guidance into daily practice.
• Support senior staff in the development of plans for the achievement of clinical excellence and demonstrate high standards of clinical practice to the team.
• As required attend case conferences, clinical reviews and CPAs.
• As required attend and/or chair other meetings and forums as directed.
• Ensure that all relevant clinical information is documented accurately and in a timely fashion onto SystmOne.
• Practice within the NMC Nurses Code of Professional Practice and Conduct at all times
• Comprehensive knowledge of legislative procedures governing care and treatment of mentally disordered offenders.
• Comprehensive knowledge of local and national policies in relation to patient confidentiality, Caldicott principles,, Child and Adult safeguarding procedures and Vulnerable Adults Policy,

Person Specification Education/Qualifications Essential

• 2 years post registration experience.
• Experience of leading and working within multi-disciplinary teams
• Experience of Prison Health Care.
• Registered Nurse.
• ENB 998 or equivalent.
• Educated to MSc level or equivalent experience

Experience Essential

• In-depth knowledge of the Care Programme Approach (CPA)
• Working in prisons or secure environments
• Knowledge of relevant documents such as nursing in Prisons and the future organisation of Prison healthcare.
• Evidence of post qualifying continued learning and awareness of developments in mental health.
• Change management skills.
• Able to translate theory into practice.
