North Staffordshire Combined Healthcare NHS Trustare looking for a compassionate, enthusiastic and motivatedBand 5 Nurse (RMN/RGN/RLDN)to join our integrated Mental Health in-reach team at Stoke Heath Prison. The role is a band 5, 37.5 hours per week and is ideal for those wanting to take the next step in career progression.
The post holder will be working within the secondary mental health in-Reach team delivering interventions with those affected by mental health, providing psychological interventions to those requiring support.
Experience and previous knowledge of mental health would be beneficial, but not essential. We will provide training, support and regular 1:1s to ensure you have the tools you need to succeed in this role.

Main duties of the job
Under the supervision and direct management of the Senior Nurse you will contribute to the provision of high quality mental health services which provides recovery focused clinical treatment for the prison population within Stoke Heath HMP.
To work within a strengths-based, recovery-orientated, change and outcomes focused approach which promotes service users as responsible co-producers of health, well-being and recovery.
To work collaboratively with other services to provide interventions to support and motivate service users and meet their needs through all stages of their individual recovery journey.
To act as named nurse / care coordinator for a specific group of prisoners and provide care under the supervision of the team manager. To deliver mental health assessment and interventions to this group of prisoners and liaise with other professionals regarding progress and outcomes.
To ensure that practice remains abreast of new developments and to develop and maintain standards and implement change in respect of this.
To liaise with other professionals and agencies both statutory and non-statutory and to provide a link between the prison and community

Job description Job responsibilities
To act as named nurse / care coordinator for an identified group of service users within the clinical caseload providing recovery focused, evidence based clinical practice to facilitate effective treatment decisions.
Engaging with the prisoners who have a known diagnosis to address their individual issues working alongside the medical and psychosocial therapies teams to improve health and social functioning.
Initiate service users safely onto clinical treatments ensuring that clinical protocols are constantly reviewed to promote best evidenced clinical practice linking to National Guidelines and Trust policies and procedures.
Undertake assessments and having the responsibility for documenting the plans of care plans ensuring they remain current and that standards of record keeping are adhered to. To take an active role in auditing and maintaining standards of record keeping for this group of prisoners.
Develop and provide care planning that supports their mental health requirements
To contribute to the seamless transition of the service users clinical needs and ensuring that care planning supports liaison and referrals to other departments involved with the service users broader care planning including:- Health Care , Offender Management, psychosocial interventions, and CMHTs based services who will take ongoing governance responsibility for prescribing treatments requirements upon release.
To have responsibility for seeking support and advice from the Senior Nurse in respect of issues and difficulties with individual clinical care. To attend regular decision making meetings with the clinical manager in order to address caseload management issues. To work as part of an MDT with in HMP Stoke Heath Prison.
To administer and manage medicines according to policy and local protocol. To assess service user response and provide advice and education to the service user in respect of prescribed medicines.
To provide support to other service users within the clinical team when the named nurse / worker is unavailable. E.g. periods of annual leave sickness etc.
Develop skills in respect of specific interventions that are appropriate to the clinical prison environment, act as a lead in developing nursing practice in relation to this.

- Registered Nurse. NMC Registration. Evidence of Continuing Professional Development.
- Qualified nursing experience in relevant areas. Evidence of good practice.
