The Swansea Crisis Resolution and Home Treatment Team (CRHTT) is a multi-disciplinary team, working in collaboration with the local Community Mental Health Teams, and In-Patients Services. The team provides a seven day service to mental health service users in the Swansea area.

The Band 6 nurse will work within the CRHTT. The Band 6 will provide specialist advice and assessments for the care of people affected by mental health problems, working with a high degree of autonomy with a specific focus on quality improvement and ensuring excellence. This role includes the provision of comprehensive telephone and face-to-face consultation services to patients and /or their carers/relatives using advanced clinical skills as part of the service delivery for unscheduled care.

The post holder is responsible for the assessing, planning, implementing and evaluating programs of care, which will enhance and maintain the quality of life and independence of service user, their families and carers. The post holder will take responsibility for specific shifts, and work within the multi-disciplinary team; its service provision and development. The role will be part of a 24 hours assessment service, and a 7 day a week home treatment team service

Person Specification Registered Nurse (RMN) Essential
  • Registered Nurse (RMN)
  • post registration experience in clinical risk assessment in acute mental health
  • understanding in best practise in acute mental health
  • Experience providing specialist advice and undertaking interview assessments
  • Able to formulate nursing risk assessment and devise intervention strategies
