We are looking for a Registered Mental Health Nurse to be part of a ground-breaking new team, improving the lives of forensic mental health service users.

Sheffield Health and Social Care NHS Foundation Trust has been chosen by NHS England to pioneer a new approach to support forensic service users in the community instead of in secure hospitals, in the form of a Specialist Community Forensic Team (SCFT). This can improve quality of life, health outcomes and reduce inequality.

As the RMN within the Specialist Community Forensic Team, you will work with a small dedicated caseload to provide quality interventions, with the support of the wider multi-disciplinary team. You will work to enhance the transition of service users from secure care settings, supporting the delivery of care and treatment, and ongoing recovery into the community. You will support the engagement in meaningful community activity, social integration and support and co-facilitate group interventions and support the vocational rehabilitation of forensic service users.

The successful candidate will be supported in their professional development, through high quality professional and clinical supervision. All members of the benefit from an extensive professional development and a range of learning events.

  • Acting as a Care Co-ordinator/Lead Professional
  • Comprehensive assessment of new referrals
  • Contributing to a duty system on a rota basis
  • Delivering supervision to more junior members of staff
  • Contribute to Multidisciplinary service development
  • Deliver Effective Medicine Management

General Requirements

Principal duties

Clinical
  • Triage and determine urgency of new referrals to the service
  • Carry out comprehensive assessments of an individuals health and social care needs in a variety of settings and agree the most appropriate course of action. This includes first presentation to service.
  • To assess, evaluate and manage clinical risk
  • To administer intramuscular medicines and ensure effective medicine management
  • To formulate a plan of care based on identified need
  • Deliver evidence-based interventions as appropriate
  • Review effectiveness of care plan at regular intervals
  • To work within safeguarding processes, including where required to act as safeguarding investigator / manager.
  • To work collaboratively with service users, carers, and partner agencies
  • To maintain computer held care records in an accurate and timely fashion
  • Undertake all work in a culturally sensitive manner
  • Act in accordance with the Care Programme Approach, Care Management System, Acute and Schedule Care Pathways as appropriate.
  • Develop effective working relationships with Primary Care colleagues


Supervision
  • Regularly review own caseload with supervisor to promote efficacy and contribute to the overall management of service
  • Deliver supervision and facilitate annual PDR with more junior members of staff
  • Participate in own PDR process, identifying training and development needs, and pursuing relevant actions to maintain and improve level of competence and effective contribution
  • Facilitate peer supervision.

Education and Training
  • Act as mentor to learners, keep up to date with relevant course and curriculum developments, and maintain sign off status
  • Participate in training relevant to role
  • Promote insight and understanding of mental health issues to service users, carers, and other agencies
  • Contribute to audit, research, and evaluation as appropriate

The job description will be reviewed regularly in the light of changing service requirements and any such changes will be discussed with the post holder.
