The successful applicants will assist in the overall delivery of care to patients within the Primary Care department of Barton Moss Secure children's services. You will be responsible for the assessment of care needs and the development, implementation and evaluation of programmes of care. The post holder will at all times comply with the NMC Code of Conduct, relevant legislation policies and procedures.

The post holder will be required to work 9am - 5pm, Monday to Friday, but flexible working may be required to meet service need. In addition there is an on call rota for new admissions - Saturday and Sunday (9am - 5pm).

This will also include some occasional cover for Marydale Lodge SCC as part of the joint healthcare contract.

Main duties of the job

To provide a Primary Healthcare provision to the young persons in the Secure Childrens Estate. This will involve screening of primary health needs, assessments, working closely with the visiting GP, liaising with the residential team whom look after the young people 24/7.

The post holder will work closely with all members of the MDT in Healthcare, developing interventions for the young people during their stay in the Homes.

You must have been in the UK for 12 months prior to applying for this post and be able to provide a 6 year address history to be able to complete the prison clearance.

  • Evidence of additional study after registration
Skills and Abilitiess Essential
  • Post holder able to provide packages of care to young people in liaison with the MDT.
  • The post holder must be able to engage with the young people to ensure effective monitoring of risk/mental health and recognise early warning signs of relapse.
  • Is able to work independently, monitoring own caseload.
Desirable
  • Has been involved in the development and implementation of change within a service.
Other Requirments Essential
  • The post holder will be required to travel between units so having the means and skills to do this is essential.
