Working in the Community Mental Health Team to provide highly specialised, high quality person centred care using advanced clinical and theoretical knowledge that is evidence based.

To plan and manage completed episodes of care within the Community Mental Health Team. Undertaking independent comprehensive assessment and management of service users to incorporate pharmacological and psychological considerations; making complex clinical decisions regarding service user management and clinical outcomes within the framework of CPA and the i-Thrive Framework.

To provide expert clinical advice, leadership and support ensure the needs of our service users are met by exercising and demonstrating high levels of clinical judgement, critical analysis and advanced decision making skills.

Main duties of the job

Develop and implement the highest quality innovative clinical practice.

To undertaken comprehensive specialist holistic assessment of service users and carers where needs are highly complex.

To work autonomously using advanced levels of history taking and assessment skills within the multidisciplinary team.

Utilise advanced clinical skills in assessment and formulate a diagnosis and treatment plan.

Be able to assess critically and re-evaluate the clinical situation as the service users condition, needs or risk changes.

Establish, maintain and effectively manage effective communication with service users, carers and professionals across health and social care .

Maintain records and compile reports maintaining confidential service user records to a very high standard reflective of Advanced Clinical Practice.

Ensure vulnerable people are safeguarded and the public are protected through the effective use of local Safeguarding procedures, MAPPA, and related procedures.

Educational responsibilities

Provide clinical placements for practitioners in training according to the nurse profession.

Be an Independent Non-Medical Prescriber able to take a history, assess, diagnose and prescribe; and develop a management including medication and monitor response to medication.

To undertaken comprehensive specialist holistic assessment of service users and carers where needs are highly complex. This will involve planning, implementing and evaluating the care delivery according to changing mental healthcare needs.

To work autonomously using advanced levels of history taking and assessment skills within the multidisciplinary team, using national and trust guidelines and protocols.

Utilise advanced clinical skills in assessment and formulate a diagnosis and treatment plan; including discharge where appropriate.

Be able to assess critically and re-evaluate the clinical situation as the service users condition, needs or risk changes.

Anticipate likely potential physical and psychological problems caused either by the condition or by the treatment and intervention plan.

Advise and communicate as appropriate with internal and external teams; including primary and social care thus ensuring seamless continuity and transfer of care, (if appropriate), for service users between other relevant health, social care and third sector agencies, professionals and other care settings.

Establish, maintain and effectively manage barriers to advance, highly skilled and effective communication with service users, carers and professionals across health and social care in order to develop a therapeutic relationship within which highly sensitive and distressing mental health problems and highly complex issues are often addressed. This includes imparting information regarding diagnosis and treatment options and referring to other teams as appropriate to promote integrated working and improve outcomes.

Take the lead in empowering and facilitating other professions to meet the mental healthcare needs of service users and carers.

Provide appropriate support and advice to carers/parents and refer for a carers assessment if appropriate.

Maintain records and compile reports maintaining confidential service user records to a very high standard reflective of Advanced Clinical Practice.

  • oRegistered Mental Health Nurse.
  • oAdvanced Nurse Practitioner.
  • oIndependent Non-medical prescriber
  • oSignificant post registration experience in mental health nursing at a senior level.
  • oExtensive, complex clinical skills and able to deliver specialised nursing care e.g. physical examination
  • oLeadership across organisation and within a team.
  • oHave an extensive portfolio of Mental Health Nursing experience, of which a significant proportion will be within Community CAMHS.
Desirable
  • oAdditional Masters degree or equivalent relevant to clinical practice
  • oPost graduate/qualification in teaching and assessing
