The post holder is a member of the In Patient Mental Health Team and will be responsible for the care coordination of a caseload of service users who are experiencing severe mental health difficulties under the supervision of the senior case worker. The post holder will take responsibility within their caseload for ensuring the assessment of health and social needs, formulation, planning, delivery and evaluation of care, support and treatment by the team, for both individuals and groups of service users including risk assessment and management, CPA, and care reviews. They will work collaboratively across a wide ranging multi-disciplinary services and agencies to provide effective delivery of health and social care, supporting a programme of recovery and social inclusion within the area.

The post holder may also supervise junior staff who are engaged with service users within their caseload.

Main duties of the job

An exciting opportunity has arisen for a RNMH/RNLD to work as part of an innovative and collaborative Psychiatric Intensive Care Unit.

The ward ethos is to provide person centred care, drawing on the individual's strengths, likes and interests, incorporating this into their care and treatment. The ward primarily supports people who are suffering levels of distress and need a secure environment, patients usually accessing our services can present with external and internal aggression, risk of absconding behaviour and high risk of self harm.

The outcome for individuals is to aid in recovery, gain an understanding of their needs, risk and reasons for behaviours.

The ward is a 9 bedded unit, comprising of both male and female service users from across Aneurin Bevan University Health Board. We shared the vision of the health board to support people in the local area as much as possible. We are a recovery focused ward that also caters for acute crisis admissions.
