Do you have a passion for working with older people? Are you committed to working with services users in their own homes, promoting skills, confidence, independence and an enhanced quality of life? Do you enjoy developing the Occupational Therapy role and service?

If so, you could be the ideal candidate for the Band 6 Community Occupational Therapist post. You will be based with the Community Mental Health Team at The Meadows in Stockport. You will be providing specific Occupational Therapy assessments and interventions to older people with functional and organic illnesses. You will be encouraged to develop your skills and interventions using evidence based practise. We have links with community resources such as Age UK andleisure services which you will also be involved in building on.

Main duties of the job

To carry out assessment and treatment.

To plan, conduct and participate in various treatment sessions in response to assessment and need.

To provide clinical and other support to multidisciplinary team.

To carry out 1:1 work and when required group sessions.

To develop and maintain community links to increase social inclusion and community integration.

To work with the ward patients and follow them up in the community.

Person Specification Education and Qualifications Essential
  • Must hold a diploma or degree in Occupational Therapy and be registered with HCPC.
  • Evidence of post qualification experience and continuing professional development to post graduate diploma level.
  • Willingness to undertake further training relevant to the role resulting from legislative or service changes, as appropriate.
Experience Essential
  • Post registration experience working in a mental health service.
  • Experience in working with people with complex mental health needs
  • Experience of MDT working
Desirable
  • Experience of working in a community setting
  • Experience of managing a caseload
  • Experience of working with older people
  • Experience of supervising students
Knowledge Essential
  • Knowledge of MOHO and other relevant assessment tools.
  • Extensive knowledge / experience in social inclusion and recovery model of care.
  • Understanding of current local and national policies in relation to Mental Health including the Mental Health Act, Risk Assessment and Management processes and CPA processes
  • Good working knowledge of assessment, care planning,
  • Comprehensive knowledge of mental health conditions and how they impact on an individual's wellbeing and occupational functioning.
  • Comprehensive knowledge of broad range of interventions required to support individuals with severe and complex mental health needs.
Desirable
  • Experience of undertaking cognitive assessment.

  • Ability to act as care coordinator for a caseload of clients.
  • Ability to work effectively as part of a MDT.
