Southampton East CMHT have an exciting new role for a passionate, enthusiastic and aspirational mental health practitioner. This role is open to registered Social Workers, Occupational Therapists and Mental Health Nurses. We are looking for someone with experience in working with people with complex mental health difficulties but can provide training within the role.

We are a fully integrated health and social care team so you would be working alongside a broad range of people from different professional disciplines including Mental health Social Workers, Peer Workers, Mental Health Nurses, Support Workers, Psychologists and Consultant Psychiatrists. As a team we support people aged 18 -- 65 with a broad range of serve and enduring mental health difficulties.

We have fantastic links with community partners and part of your role would be to engage with these services to link the people you are supporting in with additional support and guidance. This may include advice on housing, benefits or courses run by the Recovery college to support understanding and management of their mental health.

Main duties of the job

To provide specialist review and assessment of specific service users and their care pathway as identified by the CMHT Senior Management Team.

To support timeliness of interventions, a recovery focused approach to care planning and robust risk assessment supporting the onward provision of care.

Job description Job responsibilities

This should include core aspects from job evaluation such as communication Analytical / Judgement skills, Planning & Organisation, responsibilities for information R&D ,

To provide evidence based mental health assessment of care and risk of adults with mental health problems in a Community Mental Health setting, involving planning and evaluating care in collaboration with others, working as an integral part of the CMHT.

To formulate and effectively communicate care and risk management plans in accordance with identified need of the individual, ensuring that these are recovery focussed and developed in collaboration with the service user and where appropriate their relatives, carers and other involved professionals.

To act as a key member of the CMHT, contributing to multi-disciplinary discussions and decision making relating to appropriate patient care.

To be proactive in ensuring identification of service users needs to ensure they receive the range of interventions identified within their care plan, this will include physical health care and monitoring.

To operate independently in making complex clinical judgements, evaluating alternative courses of action and managing and advising on complex clinical issues regarding the treatment and care of patients.

To work with relevant agencies, procedures and legislative requirements, in particular, The NHS and Community Care Act, Mental Health Act, Data Protection Act, Adult & Child Protection Procedures, working in accordance with all relevant policies and procedures including care planning, risk assessment and safeguarding.

To participate fully in caseload supervision and management, to ensure that care plans are matched to need.

To act in a manner to respect the customs, individuality, values, sexuality and spiritual beliefs, actively supporting the individual to fulfil these.

To accurately record all contacts with service users in the appropriate Electronic Patient Record within Southern Health NHS Foundation Trust and University Hospital Southampton in line with SHFT record keeping policy and service contracting arrangements.
