Surrey Heartlands and Frimley ICS are one of the early implementer sites for community mental health transformation. The adoption of ARRS Mental Health Practitioners (MHP) is an exciting and new development that will build on and complement the progress of the new model of primary mental health care, i.e., GP Integrated Mental Health Services (GPimhs) and Mental Health Integrated Community Services (MHICS). The ARRS MHP represents an expansion and opportunity for better joint working across primary care and adult secondary care mental health systems.

Successful candidates must be experienced mental health practitioners and have experience of working in Primary Care and/or Community Mental Health Team setting. We are seeking to fill one new Full-Time(37.5 Hrs OR 5 Days per week, working from the GP Practice/s), PCN-based, Mental Health Practitioner post in a PCN within Frimley South area through Additional Roles Reimbursement Scheme (ARRS).

Main duties of the job

• To take a leadership role in supporting the adoption of the GPimhs model for those PCNS who are to launch the new service.
• To take a leadership role supporting and providing supervision and mentorship to Band 6 ARRS Mental Health practitioners and Graduate Mental Health Wellbeing Practitioners within a cluster of services (local PCNs).
• To provide a lead role in supporting the PCN in triaging mental health concerns raised by local agreed pathways at local PCN level, providing highly-specialist advice and taking responsibility for the development and implementation of specialist mental health training to the PCN.
• To provide a navigation role to help GPs make appropriate referrals to the relevant community and secondary care services bridging the gap between primary care services such as IAPT and secondary care, working in partnership across the system.

Key Responsibilities

• To be confident working as the lead locality ARRS mental health practitioner with remote support from local GPimhs or MHICS.
• To represent the ARRS MHP roles is system facing development meetings.
• To review patients presenting with mental health issues, consider a range of options regarding treatment interventions providing them with highly specialized advice concerning care when appropriate and liaising with GPs and the GPimhs team.
• To act as the first point of contact for any clinical concerns raised by any GPs and ARRS MHP, to offer advice, support and signposting to appropriate agencies.
• To receive and communicate sensitive information to clients in situations which may be highly emotive, and where barriers to acceptance and understanding may need to be overcome, requiring developed communication and empathic skills.
• To carry, manage and be responsible for own workload and the quality of care they deliver within the clinical area that he or she is responsible for, liaising regularly with GPs, practice staff GPimhs and other relevant organizations, and undertake administrative tasks related to caseload where necessary.
• To maintain and update patient records on the computer in accordance with practice policies and professional guidelines e.g., NICE, and other evidence-based guidelines.
• To liaise with appropriate resources when necessary (e.g., Welfare Rights, Housing Agencies, Voluntary Agencies, Day Services etc.)
• To ensure effective lines of communication are maintained with GPimhs to promote good working practices and ensure that service user information is passed on appropriately to other services where required.
• To participate in professional meetings as necessary where there is notification of serious concern i.e., Safeguarding and risk management concerns or in multi-disciplinary reviews.
• Subject to reasonable adjustments in respect of any disability, the post holder must possess a valid driving license, be able to always drive and have the unrestricted use of a car or motorcycle for work.

Person Specification Qualifications Essential

• Diploma/Degree or any other post graduate qualification or equivalent experience
• Qualification in Mental Health Nursing, or Occupational Therapy or Social Work
• Professional registration

Experience Essential

• Completion of mentorship / preceptorship course or in the process of completion.
• Significant post registration experience of providing a range of evidence-based interventions to people with a variety of mental health problems and serious mental illness
• Experience of coaching (supervision and appraisal of other junior clinicians)
• Demonstrable expertise in complex assessment of people with mental health issues
• Experience of leading clinical practice.
