We are looking for Mental Health Practitioners to join our responsive Community Crisis Team, whether you are a registered Mental Health Nurse, Social Worker or Occupational Therapist -- we want to hear from you!

Working in Portsmouth offers a diverse inner-city client base where we provide intensive treatment and interventions to individuals who present with a range of mental health needs as a 24 hour service. We need you to travel across the city to visit our patients, either by bike or bus, car or train! We also have pool cars which can be used by people with full UK driving licences and if you want to use your own, you can claim mileage!

Our aim is to prevent hospital admission and to be the access point for urgent mental health care to adults across Portsmouth. We work alongside our neighbouring teams to ensure patients who are in crisis are offered practical alternatives to hospital admission by the right professional at the right time.

Main duties of the job
Assess patient/client, plan develop/implement/evaluate programmes of care and carry out nursing procedures to achieve best health outcome for patient/client/family/health needs as well as actively promote health and wellbeingEvaluate clinical outcome and report variance to clinical pathway, protocols, or best practice guidelinesContribute to effective discharge planning and/or referral process to health care provider and support agencies to meet identified health needsApply diagnostic reasoning and professional judgement to nursing practiceManage daily care of patient/client and when required manage care in the absence of senior colleaguesFoster inquiry and critical thinking amongst colleagues to advance nursing practice and patient/client care to ensure the provision of positive patient/client outcomes and person-centred careAccurately document assessments of patients/client's health status, diagnosis and response to nursing and other interventions and treatmentsEffectively manage clinical care and area through the implementation of clearly defined policies and proceduresEducate and provide information to patients/clients/family to improve knowledge of illness, self-management prevention of complications and promotion of recoveryRole model safe nursing practice and report poor practice when observedPractice in accordance with legal, ethical, culturally safe, and professional standards including the Code of practice

Safeguarding
  • Identify when a child may be at risk & know how to refer to the local multi- agency safeguarding hub (MASH) when appropriate
  • Identify when a vulnerable adult may be at risk & know how to refer to the local authority adult safeguarding service

Communication
  • Provides and receives complex, sensitive information to patients/relatives and colleagues and with empathy, persuasion and reassurance and ensure information is clearly understood, where there may be barriers to understanding
  • Ensures effective collaboration and communication with multidisciplinary team, patients, carers, family and significant others, always seeking consent of the client in doing so and in co-ordinating care and fostering effective working relationships to achieve best health outcomes
  • Actively listens, drawing out information and checks understanding.
  • Empathises with others and considers their needs and feelings.
  • Expresses information effectively and accurately, both orally and in writing, adjusting language and style to recipients and considers their frame of reference.

  • Provide day to day clinical supervision of junior colleagues and students including safeguarding supervision when appropriate.

Management and Leadership
  • The post holder to ensure they work within professional boundaries
  • Staff are managed and standards of work are maintained and trust policies for the management of staff performance are adhered too.
  • Appraisals of staff directly managed are undertaken and that the appraisal process is maintained throughout the team.
  • Provide leadership role model and demonstrate specialist knowledge and high standards of clinical practice
  • Manage staff sickness absence according to policies set out locally
  • Co-ordinate day to day management of the clinical area
  • Participate in recruitment of other healthcare professionals where appropriate
  • Staff receive appropriate health and safety, and other mandatory training as prescribed and relevant records are maintained

Quality and Service Improvement
  • Identifies situations of clinical risk and takes appropriate action to ensure a safe environment for patients/clients/families and staff.
  • Contributes to evidence-based nursing practice in speciality areas.
  • Demonstrates commitment to quality improvements, risk management and resource utilisation participating in monitoring and evaluation activities including audit and research activities
  • Provides input into clinical standards/protocols and policies and undertakes clinical audits as required.
