12 Month Fixed Term/Secondment
This is an exciting opportunity for an Occupational Therapist to act as an effective member of a new pilot project delivered by Greenwich Community Mental Health Rehabilitation & Enablement Service (CMHRES). The post holder will have responsibility for delivering various interventions for service users who have complex mental health and rehabilitation needs, whilst supporting housing providers to develop their skills of delivering effective interventions to complex patients. The mainaim of the Team is to focus on rehabilitation, engagement, relapse prevention and enablement to optimise the service users' ability to live independently and enjoy a good quality of life.
You will be part of a small team working towards improving the quality of patients' lives through rehabilitation, relapse prevention, education and advice on the warning signs of relapse. You will be working alongside and with housing providers in the royal borough of Greenwich as a specialist service in order to identify and promote the most efficient use of services for service users through targeted interventions, case work advice and consultancy.
Main duties of the job
You will be working alongside the existing team to deliver the required interventions. You will be required to liaise with the CMHT based at the Heights and Ferryview to ensure continuity of care and to communicate and feedback interventions, risk management and report concerns as well as progress. You will also be required to work with the inpatient settings to ensure a smooth transition from hospital to housing provider. Maintaining appropriate, accurate written and computerised records is essential to ensure the safety of the service users and staff.
Assist in the identification, provision, and support the housing providers selected to improve the interventions available to the client group under their service.
This post is for 1 year with a review after 9 months to review interventions and patient outcomes to determine if the pilot project will be extended.To be responsible for a complex clinical caseload using evidence-based practice to assess, treat and evaluate interventions.
Provide clinical leadership and actively support the ongoing development Occupational Therapists within the service.Play a crucial role with the multi-professional team in shaping service provision.
Develop and sustain partnerships with third sector organisations to enhance the recovery pathway of service users as they progress from an inpatient setting into the community by instilling hope and having a genuine impact on their quality of life.
